参芪复脉汤联合通心络胶囊治疗急性ST段抬高型心肌梗死疗效研究

摘　　要：目的探讨参芪复脉汤联合通心络胶囊治疗急性ST段抬高型心肌梗死的临床疗效,以及对患者心肌酶谱指标的影响。方法选取河北省香河县人民医院2017年1月至2018年10月收治的急性ST段抬高型心肌梗死患者80例,随机分为观察组和对照组,各40例。两组患者均予常规西药治疗,观察组患者加用参芪复脉汤联合通心络胶囊,均治疗4周。结果观察组总有效率为92.50%,显著高于对照组的75.00%(P<0.05)。治疗后,两组患者左室射血分数、左室短轴缩短率均显著升高,左心室舒张末期内径、左室收缩末期内径均显著降低,且观察组改善更明显(P<0.05);两组血清心肌肌钙蛋白T、肌酸激酶MB型同工酶、核因子κB水平均显著降低,且观察组降低更显著(P<0.05);两组患者血管内皮素1、血管性血友病因子水平均显著降低,血管内皮生长因子水平均显著升高,且观察组改善更显著(P<0.05)。治疗期间,观察组和对照组不良反应发生率相当(15.00%比10.00%,P>0.05)。结论参芪复脉汤联合通心络胶囊治疗急性ST段抬高型心肌梗死临床疗效较好,能改善患者的心功能、心肌酶谱、血管内皮功能指标水平,且安全性良好。Objective To investigate the clinical efficacy of Shenqi Fumai Decoction combined with Tongxinluo Capsules in the treatment of acute ST-segment elevation myocardial infarction and its effect on the myocardial enzyme spectrum.Methods A total of 80 patients with acute ST-segment elevation myocardial infarction admitted to the Xianghe People's Hospital from January 2017 to October 2018 were selected and randomly divided into the observation group and the control group,40 cases in each group.The patients in the two groups were treated with conventional Western medicine,on this basis,the patients in the observation group were treated with Shenqi Fumai Decoction combined with Tongxinluo Capsules.Both groups were treated for four weeks.Results The total effective rate of the observation group was 92.50%,which was significantly higher than 75.00%of the control group(P<0.05).After treatment,the left ventricular ejection fraction(LVEF)and left ventricular short-axis fractional shortening(FS)in the two groups increased significantly,while the left ventricular end-diastolic diameter(LVEDd)and left ventricular end-systolic diameter(LVESd)in the two groups decreased significantly,and the improvement in the observation group was more significant(P<0.05).After treatment,the level of serum cardiac troponin T(cTnT),creatine kinase isoenzyme(CK-MB),nuclear factor-κB(NF-κB)in the two groups decreased significantly,and the decrease in the observation group was more significant(P<0.05).The levels of vascular endothelin-1(ET-1)and von Willebrand factor(v WF)decreased significantly,while the level of vascular endothelial growth factor(VEGF)increased signifi-cantly in the two groups,and the improvement in the observation group was more significant(P<0.05).During the treatment,the in-cidence of adverse reactions in the observation group was similar to that in the control group(15.00%vs.10.00%,P>0.05).Conclusion Shenqi Fumai Decoction combined with Tongxinluo Capsules are effective and safe in the treatment of acute ST-segment elevation
