摘 要:目的检测广州市北部地区健康妊娠女性凝血指标水平,建立不同孕期凝血指标的参考区间。方法选取2019年6月至2021年6月在广州市中西医结合医院建档的健康孕妇2 527例,分为早孕组(≤13周,868例);中孕组(> 13-28周,822例);晚孕组:(> 28周,837例),以同期124名健康育龄期女性作为非孕对照组。采用百分位数法(P_(2.5)-P_(97.5))确定妊娠早、中、晚期女性活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)、凝血酶时间(TT)和纤维蛋白原(Fib)的参考区间。结果与非孕对照组相比,随着孕周的进展Fib水平有较大程度的升高,PT水平在孕早、中、晚期较对照组有缩短趋势,TT水平在孕晚期较孕早期、孕中期显著下降,差异均有统计学意义(P <0.05)。整个孕期APTT水平的变化并不显著(P> 0.05)。结论本次研究初步建立了适用于本实验室的不同孕期常规凝血指标参考区间,提高了凝血功能在妊娠期的应用价值。Objective The coagulation indexes of healthy pregnant women in northern Guangzhou were measured to establish the reference interval of coagulation indexes of healthy pregnant women in different pregnancies. Methods A total of 2 527 healthy pregnant women registered in Guangzhou Hospital of Integrated Chinese and Western Medicine from June 2019 to June 2021 were selected and divided into the early trimester pregnancy group(868 cases), the second-trimester pregnancy group(822 cases) and the third-trimester pregnancy group(837 cases) according to pregnancy period, with 124 healthy reproductive-age women selected as the non-pregnant control group in the same period. The percentile method(P_(2.5)-P_(97.5)) was used to determine the reference intervals of activated partial thromboplastin time(APTT), prothrombin time(PT), thrombin time(TT), and Fibrinogen(Fib).Results Compared with the non-pregnant control group, with the progress of pregnancy period, the PT level has a tendency to shorten in the early, second,and third-trimester pregnancy. The TT level in the third-trimester pregnancy was significantly lower than that in the early and second-trimester pregnancy,and the differences were statistically significant(P<0.05). Changes in APTT level during pregnancy was not significant(P>0.05). Conclusion This study preliminarily established the reference interval of routine coagulation indexes for healthy pregnant women applicable to our laboratory, which can improve the application value of coagulation function in pregnancy.
