结石CT值对经尿道输尿管软镜钬激光碎石治疗肾结石的影响

摘　　要：目的讨论结石CT值对经尿道输尿管软镜钬激光碎石治疗肾结石碎石效率、结石清除率及并发症的影响。方法将2019年2月至2021年5月于我院泌尿外科行经尿道输尿管软镜钬激光碎石治疗的100例肾结石患者作为本次研究对象。所有患者均接受术前CT检查,获取CT值,均接受经尿道输尿管软镜钬激光碎石治疗,并在术后以CT检查结石清除率。结果 100例入选患者术前结石CT值均值为(969.5±237.8)Hu,其中结石CT值<1 000 Hu与≥1 000 Hu的分别有55例(55.00%)、45例(45.00%),将其分为甲组及乙组。甲组及乙组患者在性别、年龄、结石大小、肾下盏结石数量及合并症方面无统计学意义,P> 0.05。乙组手术时间明显长于甲组,统计学对比差异显著,P <0.05;甲组及乙组术中、术后并发症及术后3个月结石清除率方面无统计学意义,P> 0.05。结论结石CT值≥1 000 Hu时将延长肾结石患者经尿道输尿管软镜钬激光碎石手术耗时,影响碎石效率,但是对术后结石清除率及并发症的影响性不大,可将术前结石CT值作为评估手术时间及碎石效率的有效指标,为制订手术计划提供科学依据。Objective To discuss the effect of stone CT value on lithotripsy efficiency, stone clearance and complications of transurethral ureteroscopic holmium laser lithotripsy in the treatment of renal calculi. Methods One hundred patients with renal calculi who underwent transurethral ureteroscopic holmium laser lithotripsy in the Department of Urology of our hospital from February 2019 to may 2021 were selected as the subjects of this study. All patients underwent preoperative CT examination, obtained CT values, received transurethral ureteroscopic holmium laser lithotripsy, and checked the stone clearance rate by CT after operation. Results The mean value of preoperative stone CT in 100 patients was(969.5±237.8) Hu, of which 55 cases(55.00%)and 45 cases(45.00%) with stone CT value<1 000 Hu and≥1 000 Hu respectively. They were divided into group A and group B. There was no significant difference in gender, age, stone size, number of lower renal calyceal stones and complications between group A and group B(P>0.05). The operation time in group B was significantly longer than that in group A(P<0.05);there was no significant difference in intraoperative and postoperative complications and stone clearance rate 3 months after operation in group A and group B(P >0.05). Conclusion When the stone CT value≥1000 Hu, it will prolong the timeconsuming of transurethral ureteroscopic holmium laser lithotripsy in patients with renal stones and affect the lithotripsy efficiency, but it has little impact on the postoperative stone clearance and complications. The preoperative stone CT value can be used as an effective index to evaluate the operation time and lithotripsy efficiency, so as to provide a scientific basis for formulating the operation plan.
