胰岛素不同给药方式治疗糖尿病酮症酸中毒的疗效观察

摘　　要：目的探讨胰岛素不同给药方式治疗糖尿病酮症酸中毒的疗效。方法将84例糖尿病酮症酸中毒患者随机分为两组各42例,研究组采取胰岛素泵持续皮下输注,对照组采取小剂量胰岛素静脉输注。记录两组患者的血糖达标时间,血、尿酮体转阴时间及低血糖发生率,同时监测患者的动脉血气分析。结果研究组的血糖达标时间为(6.73±1.24)h,尿酮体转阴时间为(6.14±2.42)h,血酮体转阴时间为(14.92±2.72)h,低血糖发生率为7.14%;对照组分别为(8.47±0.83)h,(15.03±3.77)h,(31.92±4.03)h,19.05%,比较有统计学意义(P<0.05)。治疗后,研究组的pH值为(7.45±0.25),剩余碱为(-4.26±0.37),HCO3-为(18.03±1.25),对照组分别为(7.33±0.17)、(-5.72±0.21)、(15.35±1.71),比较有统计学意义(P<0.05)。结论糖尿病酮症酸中毒患者采取胰岛素静脉微泵持续皮下输注能够更快地使血液达标、酮体转阴,纠正动脉血气分析,有效降低低血糖发生率。Objective To explore the efficacy of different insulin delivery methods in the treatment of diabetic ketoacidosis.Methods A total of 84 patients with diabetic ketoacidosis were randomly divided into two groups with 42 patients each.The time of blood glucose reaching the standard,the time of urine ketone body turning negative and the incidence of hypoglycemia in the two groups were recorded,and the arterial blood gas analysis of the patients was monitored.Results In the study group,the time of blood glucose reaching the standard was(6.73±1.24)h,the time of urine ketone body turning negative was(6.14±2.42)h,the time of blood ketone body turning negative was(14.92±2.72)h,and the incidence of hypoglycemia was 7.14%.The control group was(8.47±0.83)h,(15.03±3.77)h,(31.92±4.03)h,and 19.05%respectively.The comparison were statistically significant(P<0.05).After treatment,the pH value of the study group was(7.45±0.25),the residual base was(-4.26±0.37),and HCO3-was(18.03±1.25),while the control group was(7.33±0.17),(-5.72±0.21),and(15.35±1.71).The comparison were statistically significant(P<0.05).Conclusion Continuous subcutaneous infusion with insulin intravenous micropump in patients with diabetic ketoacidosis can make blood reach the standard faster,turn ketone body negative,correct arterial blood gas analysis,and effectively reduce the incidence of hypoglycemia.
