多切口减压+VSD持续负压吸引治疗急性坏死性筋膜炎的临床应用研究

摘　　要：目的研究多切口减压+VSD持续负压吸引治疗急性坏死性筋膜炎的临床疗效。方法选取2019年2月-2021年2月我院诊治的40例急性坏死性筋膜炎患者为研究对象,采用随机数字表法分为对照组和观察组,各20例。对照组采用多切口减压治疗,观察组在对照组基础上联合VSD持续负压吸引治疗,比较两组治疗总有效率、临床治疗指标、疼痛评分、并发症发生率。结果观察组治疗总有效率为85.00%,高于对照组的75.00%(P<0.05);观察组创口愈合时间、换药次数、住院时间均小于对照组(P<0.05);观察组疼痛评分低于对照组(P<0.05);观察组并发症发生率为10.00%,低于对照组的20.00%(P<0.05)。结论多切口减压联合VSD持续负压吸引治疗急性坏死性筋膜炎效果确切,可提高临床治疗总有效率,促进创口快速愈合,减少换药频率,缩短周期,减轻患者疼痛度,降低并发症发生率,是一种可行、有效的治疗方案。Objective To study the clinical efficacy of multi-incision decompression and VSD continuous negative pressure suction in the treatment of acute necrotizing fasciitis.Methods Forty patients with acute necrotizing fasciitis who were treated in our hospital from February 2019 to February 2021 were selected as the research objects.They were divided into control group and observation group by random number table method,with 20 cases in each group.The control group was treated with multi-incision decompression,and the observation group was treated with VSD continuous negative pressure suction on the basis of the control group.The total effective rate,clinical treatment index,pain score and complication rate of the two groups were compared.Results The total effective rate of the observation group was 85.00%,which was higher than 75.00%of the control group(P<0.05).The wound healing time,dressing change times and hospitalization time in the observation group were less than those in the control group(P<0.05).The pain score in the observation group was lower than that in the control group(P<0.05).The incidence of complications in the observation group was 10.00%,which was lower than 20.00%in the control group(P<0.05).Conclusion Multi-incision decompression combined with VSD continuous negative pressure suction is effective in the treatment of acute necrotizing fasciitis,which can improve the total effective rate of clinical treatment,promote rapid wound healing,reduce the frequency of dressing change,shorten the cycle,reduce the pain and the incidence of complications.It is a feasible and effective treatment plan.
