摘　　要：目的分析改良经尿道前列腺电切术(TURP)对体积>80mL良性前列腺增生患者逼尿肌稳定性的影响。方法选取2018年8月至2020年8月开滦总医院诊治的90例体积>80mL良性前列腺增生患者作为研究对象。将采用TURP的45例患者纳入常规组,采用改良TURP的45例患者纳入改良组。观察患者逼尿肌稳定性,评价患者临床症状、生活质量,观察手术相关指标及术后并发症情况。结果两组术后逼尿肌稳定性相关指标均高于术前,且改良组高于常规组(P<0.05)。两组术后6个月IPSS评分低于术前,QOL评分高于术前;且改良组IPSS评分低于常规组,QOL评分高于常规组(P<0.05)。改良组术中出血量少于常规组,住院时间、导尿管留置时间短于常规组(P<0.05)。改良组术后并发症发生率低于常规组(P<0.05)。结论改良TURP能显著提高体积>80mL良性前列腺增生患者逼尿肌稳定性,改善患者临床症状,提高其生活水平,减少手术并发症,手术效果显著。Objective To analyze the effect of modified transurethral resection of prostate(TURP) on detrusor stability in patients with BPH>80 mL. Methods 90 patients with benign prostatic hyperplasia(BPH) with volume>80 mL treated in Kailuan General Hospital from August 2018 to August 2020 were selected as the study subjects. 45 patients receiving TURP were enrolled in the conventional group and 45 patients receiving modified TURP were enrolled in the modified group. The stability of the patient′s detrusor muscle was observed, the clinical symptoms and quality of life were evaluated, and the operative indicators and postoperative complications were observed. Results The stability related indexes of detrusor in both groups were higher than those before operation, and those in modified group were higher than those in conventional group(P<0.05). IPSS score 6 months after operation was lower than that before operation, and QOL score was higher than that before operation. The IPSS score of the modified group was lower than that of the conventional group, and the QOL score was higher than that of the conventional group(P<0.05). The amount of intraoperative blood loss in modified group was less than that in conventional group, and the length of hospital stay and indwelling catheter time were shorter than that in conventional group(P<0.05). The incidence of postoperative complications in modified group was lower than that in conventional group(P<0.05). Conclusions Modified TURP can significantly improve the stability of detrusor in patients with BPH>80 mL, improve the clinical symptoms and the living standard of patients, and reduce surgical complications.
