摘　　要：目的通过Meta分析比较腹腔镜手术和开腹手术治疗休克型异位妊娠患者的有效性及安全性。方法通过中国知网(CNKI)、PubMed等中英文数据库,以"异位妊娠""休克""腹腔镜"等关键词进行检索,文献类型为随机对照研究(RCT),检索时间截止至2020年7月30日。由2名研究者单独进行文献筛选、提取资料及质量评价。使用RevMan 5.4和Stata15.0进行Meta分析。结果有18篇共1 576例患者纳入研究。腹腔镜手术患者的术中出血量[SMD=-1.53,95%CI(-1.67,-1.39)]、手术时间[SMD=-0.63, 95%CI(-0.75,-0.50)]、术后疼痛[RR=0.28, 95%CI(0.21,0.36)]、术后感染[RR=0.31, 95%CI(0.10, 0.93)]均低于开腹手术患者,差异具有统计学意义(P<0.001)。结论对休克型异位妊娠患者而言,腹腔镜手术能够减少术中出血量,缩短手术时间,减轻术后疼痛程度,减少术后并发症的发生。Objective To compare the effectiveness and safety of laparoscopic surgery and open surgery in the treatment of shock-type ectopic pregnancy patients by Meta-analysis. Methods Keywords such as "ectopic pregnancy" "shock" and "laparoscopy" were searched through CNKI, PubMed and other Chinese and English databases by the randomized controlled study(RCT), and the search time was up to July 30, 2020. Literature screening, data extraction and quality evaluation were carried out by two researchers. Meta-analysis was performed using RevMan 5.4 and Stata15.0. Results A total of 18 articles with 1,576 patients were included in the study. Intraoperative blood loss [SMD=-1.53, 95%CI(-1.67,-1.39)], operation time [SMD=-0.63, 95%CI(-0.75,-0.50)], postoperative pain [RR=0.28, 95%CI(0.21, 0.36)], postoperative infection [RR=0.31, 95%CI(0.10, 0.93)] of laparoscopic surgery patients were lower than those of patients undergoing open surgery(P<0.001). Conclusions For patients with shock-type ectopic pregnancy, laparoscopic surgery can reduce intraoperative blood loss, shorten operation time, reduce postoperative pain and postoperative complications.
