摘 要:目的明确晚期糖基化终末产物受体(RAGE)与主动脉瓣钙化炎症机制、骨化机制的相关性。方法收集2018年2月至2018年5月天津市胸科医院收治的主动脉瓣钙化患者11例为钙化组,同期体检中心健康体检者11例为对照组。运用Elisa检测AGEs、sRAGE、TNF-α、IL-1β、IL-6在钙化主动脉瓣患者血清中的表达情况,明确RAGE与主动脉瓣钙化过程中成骨机制的相关性(应用Western Blot、RT-PCR、免疫组化检测RAGE、BGP、BMP-2、TGF-β在钙化主动脉瓣中的表达情况)。结果钙化组患者血清中的AGEs、TNF-α、IL-1β、IL-6较对照组患者明显上升[(70.29±16.22)nmol/L比(43.50±9.56)nmol/L,(42.08±11.06)ng/L比(22.76±5.51)ng/L,(39.79±8.66)ng/L比(16.37±2.72)ng/L,(52.43±11.06)ng/L比(28.37±4.93)ng/L,P<0.05],sRAGE较对照组患者明显下降[(87.80±9.62)ng/L比(107.61±10.44)ng/L,P<0.05]。钙化主动脉瓣样本中定量PCR检测和Western试验提示,RAGE、BGP、BMP-2、TGF-β明显高于非钙化主动脉瓣(P<0.01)。免疫组化提示,钙化组RAGE、BGP、BMP-2、TGF-β较非钙化组明显高表达。结论 RAGE可能通过参与炎症反应及成骨作用促进主动脉瓣钙化。Objective To explore the correlation between(RAGE) and inflammatory mechanisms and ossification of aortic valve calcification. Methods 11 patients with aortic stenosis calcified in Tianjin chest hospital from February 2018 to May 2018 and 11 healthy subjects in the physical examination center in the same period were collected. To clarify the correlation between RAGE and inflammatory mechanisms involved in aortic valve calcification, ELISA was used to examine the expression of AGEs, s RAGE, TNF-α, IL-1 and IL-6 in the serum of patients with calcified aortic valves. To clarify the relevance of RAGE to the osteogenic mechanism during aortic valve calcification Western blot, RT-PCR and immunohistochemistry were used to detect the expression of BGP, BMP-2 and TGF-β in calcified aortic valve. Results AGEs and TNF-α、IL-1β and IL-6 in the serum of calcified group were significantly higher than that in the control group [(70.29 ± 16.22)nmol/L vs.(43.50±9.56)nmol/L,(42.08±11.06)ng/L vs.(22.76±5.51)ng/L,(39.79±8.66)ng/L vs.(16.37±2.72)ng/L,(52.43±11.06)ng/L vs.(28.37±4.93)ng/L,P<0.05]. And sRAGE was significantly lower than that in the control group [(87.80 ± 9.62)ng/L vs.(107.61 ± 10.44)ng/L,P<0.05]. PCR and Western Blot in calcified aortic valve samples showed that RAGE, BGP, BMP-2 and TGF-β were significantly higher than that in non calcified aortic(P<0.01). Immunohistochemistry showed that RAGE, BGP, BMP-2 and TGF-β in calcified aortic were significantly higher expression than that in non calcified aortic. Conclusions RAGE may promote aortic valve calcification by participating in the inflammatory response and osteogenesis.
