摘  要:目的探讨风湿性心脏病(RHD)患者瓣膜组织中微管相关蛋白轻链3-Ⅱ(LC3-Ⅱ)和骨唾液蛋白(BSP)与胶原容积分数(CVF)的相关性及发病危险因素。方法选取行手术切除的二尖瓣瓣膜患者260例,其中RHD患者188例(RHD组),非RHD患者72例(非RHD组)。采用Western blot法检测瓣膜组织中LC3-Ⅱ和BSP水平,采用Masson法进行胶原纤维染色,并检测CVF。比较2组LC3-Ⅱ、BSP、CVF水平,采用Pearson相关分析LC3-Ⅱ、BSP与CVF的相关性,用ROC曲线分析LC3-Ⅱ和BSP对RHD的诊断价值,采用logistic回归分析发生RHD的独立危险因素。结果 RHD组LC3-Ⅱ、BSP、CVF水平明显高于非RHD组,差异有统计学意义(P<0.01)。Pearson相关性分析显示,LC3-Ⅱ、BSP与CVF呈正相关(r=0.642,r=0.607,P<0.01)。ROC曲线分析显示,LC3-Ⅱ诊断RHD的曲线下面积为0.738(95%CI:0.694~0.787),截断值为0.55。BSP诊断RHD的曲线下面积为0.759(95%CI:0.708~0.803),截断值为0.86。LC3-Ⅱ和BSP联合诊断RHD的曲线下面积为0.841(95%CI:0.796~0.885),敏感性、特异性分别为94.68%和76.39%。logistic回归分析显示,LC3-Ⅱ、BSP、高血压、年龄以及地区均是发生RHD的影响因素(P<0.05,P<0.01)。结论 RHD患者瓣膜组织中LC3-Ⅱ和BSP均呈高表达,与CVF相关,是RHD发生的独立危险因素,LC3-Ⅱ和BSP联合检测具有较高的诊断效能。Objective To study the correlation of microtubule-associated protein 1 light chain 3-Ⅱ(LC3-Ⅱ) and bone sialoprotein(BSP) expressions with collagen volume fraction(CVF) in valvular tissue of rheumatic heart disease(RHD) patients and the risk factors for RHD.Methods Two hundred and sixty patients who underwent surgical resection of mitral valve in our hospital from August 2017 to August 2019 were divided into RHD group(n=188) and RHD-free group(n=72).The expressions of LC3-Ⅱ and BSP in mitral valve tissue were detected by Western blot, and the CVF in collagen fibers was measured with Masson staining.The LC3-Ⅱ and BSP expressions and CVF were compared between the two groups.The correlation of LC3-Ⅱand BSP with CVF in RHD patients was analyzed by Pearson correlation analysis.The value of LC3-Ⅱ and BSP expressions for diagnosing RHD was analyzed by ROC curve analysis.The independent risk factors for RHD were analyzed by logistic regression analysis.Results The LC3-Ⅱand BSP expression levels and CVF were significantly higher in RHD group than in RHD-free group(P<0.01).Pearson correlation analysis showed that the LC3-Ⅱ and BSP expressions were positively correlated with the CVF in RHD patients(r=0.642,r=0.607,P<0.01).ROC curve analysis displayed that the ACU of LC3-Ⅱ for diagnosing RHD was 0.738(95%CI:0.694-0.787) with an optimal cut-off value of 0.55.The AUC of BSP for diagnosing RHD was 0.759(95%CI:0.708-0.803) with an optimal cut-off value of 0.86.The AUC of LC3-Ⅱcomined with BSP for diagnosing RHD was 0.841(95%CI:0.796-0.885) with a sensitivity of 94.68% and a specificity of 76.39%.Logistic regression analysis revealed that LC3-Ⅱ,BSP,hypertension, age and region were risk factors for RHD(P<0.05,P<0.01).Conclusion Upregulated LC3-Ⅱ and BSP expressions in mitral valve tissue are positively correlated with CVF in RHD patients and are an independent risk factor for RHD.Combined detection of LC3-Ⅱ and BSP expressions can effectively diagnose RHD.
