摘 要:目的探讨无创呼吸机联合高流量氧疗治疗慢性阻塞性肺疾病急性加重期(AECOPD)合并呼吸衰竭患者疗效及对血清谷氨酰转肽酶(GGT)、血清胱抑素-C(CysC)及超敏C反应蛋白(hs-CRP)水平的影响。方法选取2018年1月至2020年1月在本院确诊的80例AECOPD合并呼吸衰竭患者,采用简单随机分组将患者分为观察组和对照组,每组各40例。对照组患者采用无创呼吸机治疗,观察组采用无创呼吸机联合高流量氧疗治疗,均持续治疗7 d。治疗后,评估两组患者的临床疗效;分别于治疗前后,采用酶联免疫吸附试验(ELISA)法测定患者GGT、CysC、hs-CRP水平,比较两组患者的GGT、CysC、hs-CRP以及血流动力学的变化;记录两组患者治疗期间的不良反应发生情况。结果观察组治疗的总有效率为90.00%,与对照组的77.50%无显著差异(P>0.05);治疗后,观察组血清GGT、CysC和hs-CRP水平均显著低于对照组(P<0.05);观察组患者血氧饱和度(SpO_(2))和动脉血氧分压(PaO_(2))水平显著高于对照组,呼吸频率(RR)水平显著低于对照组(P<0.05);观察组的总不良反应发生率是10.00%,相较于对照组的20.00%无显著差异(P>0.05)。结论无创呼吸机联合高流量氧疗治疗AECOPD合并呼吸衰竭患者的疗效确切,可改善患者的血流动力学指标水平,降低血清GGT、CysC、hs-CRP水平,安全性高。Objective To investigate the efficacy of non-invasive ventilator combined with high-flow oxy-gen therapy in the treatment of patients with acute exacerbation of chronic obstructive pulmonary disease(AECO-PD)complicated with respiratory failure and the effect on serum glutamyl transpeptidase(GGT)and serum cystatin-C(CysC)And the level of high-sensitivity C-reactive protein(hs-CRP).Methods A total of 80 AECOPD patients with respiratory failure diagnosed in our hospital from January 2018 to January 2020 were selected.The pa-tients were divided into observation group and control group by simple random grouping,with 40 cases in each group.The control group was treated with a non-invasive ventilator,and the observation group was treated with a non-invasive ventilator combined with high-flow oxygen therapy,and the treatment was continued for 7 days.Af-ter treatment,the clinical efficacy of the two groups of patients was evaluated;before and after treatment,the levels of GGT,CysC,and hs-CRP were measured by enzyme-linked immunosorbent assay(ELISA),and the GGT,CysC,hs-CRP and levels of the two groups were compared.Changes in hemodynamics;record the occurrence of adverse reactions during the treatment of the two groups of patients.Results The total effective rate of treatment in the observation group was 90.00%,which was not significantly different from 77.50%in the control group(P>0.05);after treatment,the serum GGT,CysC and hs-CRP levels in the observation group were significantly lower than those in the control group(P<0.05);the observation group's blood oxygen saturation(SpO_(2))and arterial par-tial pressure(PaO_(2))levels were significantly higher than those of the control group,and the respiratory rate(RR)level was significantly lower than that of the control group(P<0.05);the observation group's total The incidence of adverse reactions was 10.00%,which was no significant difference compared to 20.00%in the control group(P>0.05).Conclusion Non-invasive ventilator combined with high-flow oxygen therapy has a definite
