摘  要:目的探讨彩色多普勒超声评价增生性瘢痕的临床价值。方法自2018年7月至2020年7月,上海交通大学医学院附属第九人民医院选取18只雄兔制作兔耳增生性瘢痕模型。分别在第0、4、8、12、24、36周检测,通过超声波评价瘢痕增生厚度、血管密度、血流速度和硬度,客观分析瘢痕增生规律。结果超声检测的数据显示,兔耳瘢痕在第24周达到增生高峰,瘢痕厚度和血管密度均最大,随后血管流速下降,瘢痕厚度不再增加,呈现萎缩趋势。瘢痕厚度变化与其内血管密度呈正相关性。瘢痕硬度在第8周开始显著增加,24周达到高峰。结论彩色多普勒超声可以客观评价瘢痕增生过程,对临床综合治疗瘢痕具有指导意义。Objective To explore the clinical value of color Doppler ultrasound in the evaluation of the hyperplastic scar.Methods From July 2018 to July 2020,18 male rabbits were selected to make rabbit ear models of hyperplastic scar.Specimens were collected at 0,4,8,12,24 and 36 weeks,respectively.The thickness,vascular density,blood flow velocity and hardness of hyperplastic scar were detected by ultrasound.The feature of scar hyperplasia was analyzed objectively.Results The data of color Doppler ultrasound showed that the hyperplasia of rabbit ear scar reached the peak at 24 weeks,with the maximum scar thickness and vascular density,followed by decrease of blood flow velocity.No increase of scar thickness was found.The change of scar thickness was correlated with the density of internal vessels.The scar hardness began to increase significantly from 8 weeks and reached a peak at 24 weeks.Conclusion Color Doppler ultrasound can objectively evaluate the process of scar hyperplasia,which has guiding significance for clinical comprehensive treatment of scar.
