摘　　要：目的研究心脏外科手术后医院感染特点及影响因素,为制订防控措施提供依据。方法通过前瞻性监测方法,对某医院心脏外科手术患者术后医院感染及其影响因素进行研究与分析。结果共分析某医院实施心脏外科手术1377例,发生术后医院感染106例,术后感染率为7.70%。复合心脏手术、主动脉手术的术后感染率最高,分别为22.92%和21.15%。术后感染部位主要是下呼吸道、血液系统和手术部位。下呼吸道和血液感染病原菌以革兰阴性菌为主,手术部位感染以革兰阳性菌为主。多因素分析结果显示,慢性肺部疾病、手术前曾感染、术前住院时间长和机械通气时间长等因素构成心外科手术术后医院感染的独立危险因素。结论心脏外科手术术后医院感染发生率较高,高风险因素较多,应针对相关危险因素采取有效的防控措施。Objective To study the characteristics and influencing factors of nosocomial infection after cardiac surgery,so as to provide a basis for formulating prevention and control measures.Methods Through prospective monitoring methods,postoperative nosocomial infection and its influencing factors were studied and analyzed in patients undergoing cardiac surgery in a hospital.Results A total of 1377 cases of cardiac surgery performed in a hospital were analyzed,106 cases of postoperative nosocomial infection occurred,and the postoperative infection rate was 7.70%.Combined heart surgery and aortic surgery had the highest postoperative infection rates,which were 22.92%and 21.15%,respectively.The postoperative infection sites were mainly the lower respiratory tract,blood system,and surgical site.The pathogenic bacteria of lower respiratory tract and blood infection were mainly Gram-negative bacteria,and the main pathogens of surgical site infection were Gram-positive bacteria.The results of multivariate analysis showed that chronic lung disease,preoperative infection,long preoperative hospital stay,long mechanical ventilation were independent risk factors for nosocomial infection after cardiac surgery.Conclusion The incidence of nosocomial infections after cardiac surgery is relatively high,and there are many high-risk factors.Effective prevention and control measures should be taken for related risk factors.
