摘　　要：目的探究血清中性粒细胞明胶酶相关性脂质运载蛋白(NGAL)对糖尿病肾病合并急性肾损伤诊断评估的临床价值。方法将2019年5月-2020年1月期间收治的糖尿病肾病合并早期急性肾损伤110例患者及同期与AKI组患者年龄、性别、居住区域匹配的110例健康体检者作为研究对象,分别作为AKI组(n=110)及对照组(n=110),将早期AKI患者分为3组,分别为1期组(n=41)、2期组(n=42)、3期组(n=37),对比各组的β_(2)微球蛋白(β_(2)-MG)及胱抑素C(Cys-C)、NGAL。结果AKI组β_(2)-MG、Cys-C、血清NGAL及尿液NGAL均较对照组高(P<0.05)。AKI组不同分期、不同病因β_(2)-MG、Cys-C、血清及尿液NGAL对比具有明显差异(P<0.05)。将AKI患者相关数据进行Pearson相关分析,结果显示,血清NGAL与β_(2)-MG(r=0.654,P<0.05)、Cys-C(r=0.754,P<0.05)均呈正相关,尿液NGAL与β_(2)-MG(r=0.685,P<0.05)、Cys-C(r=0.813,P<0.05)均呈正相关,(P<0.05)。结论分期越高,β_(2)-MG、Cys-C、血清NGAL水平及尿液NGAL水平越高,可以评估病情,具有较高的应用价值。Objective To investigate the clinical value of NGAL in the diagnosis and evaluation of diabetic nephropathy complicated with acute kidney injury.Methods 110 patients with diabetic nephropathy complicated with early acute kidney injury admitted from May 2019 to January 2020 and 110 healthy subjects matched with age,sex and residential area of patients in the AKI group during the same period were selected as study objects.Patients with early AKI were divided into 3 groups,namely the stage 1 group(n=41),the stage 2 group(n=42)and the stage 3 group(n=37).Theβ_(2)-MG、Cys-C and NGAL of each group were compared.Results AKI group 2-mg,CYS-C,serum NGAL and urine NGAL were higher than the control group(P<0.05).AKI was significantly different in different stages,differentβ_(2)-MG、Cys-C,serum and urine NGAL(P<0.05).Pearson correlation analysis of the data before treatment showed that serum NGAL was positively correlated withβ_(2)-MG(r=0.654,P<0.05)andCys-C(r=0.754,P<0.05),while urine NGAL was positively correlated withβ_(2)-MG(r=0.685,P<0.05)and Cys-C(r=0.813,P<0.05),can evaluate the disease,which has high application value.
