活动平板试验在男性冠心病中的诊断价值

摘　　要：目的评价活动平板心电图负荷试验(TET)在男性冠心病中的诊断价值。方法随机选取2018年1月至2020年1月间我院收治的120例可疑冠心病男性患者。受试者先后行活动平板心电图负荷试验和冠脉造影检查(CAG)。先按年龄进行分层(<50岁、50~60岁、>60岁),再评估活动平板试验的敏感度、特异度,最后了解△ST/△HR指数法能否提高活动平板心电图负荷试验的敏感度及特异度。结果在男性冠心病中,活动平板试验的敏感度为76.67%,特异度为53.33%。在不同年龄亚组中,真阳性率依次为96.15%(<50岁亚组)、86.67%(50~60岁亚组)与66.67%(>60岁亚组),不同年龄组间比较差异有统计学意义(P<0.05)。此外,△ST/△HR指数能够提高活动平板试验的敏感度及特异度。结论活动平板试验对男性冠心病的诊断具有一定的价值。随着年龄增加,活动平板心电图负荷试验的特异性相应降低。ST段压低程度结合心率增快程度(即△ST/△HR)能提高活动平板心电图负荷试验的敏感度及特异度,在男性冠心病诊断过程中具有重大意义。Objective To evaluate the diagnostic value of electrocardiogram treadmill exercise test(TET)of male coronary heart disease.Methods A total 120 male patients with suspected coronary heart disease admitted to our hospital from January 2018 to January 2020 were randomly selected.They were given electrocardiogram treadmill exercise test followed by coronary angiography(CAG).The patients were stratified according to age(<50 years old,50-60 years old,>60 years old).The sensitivity and specificity of treadmill exercise test were evaluated.Analysis was conducted to investigate whether the△ST/△HR index method can improve the sensitivity and specificity of treadmill exercise test.Results In men with coronary heart disease,the sensitivity of treadmill exercise test was 76.67%and the specificity was 53.33%.In different age subgroups,the true positive rate was 96.15%(<50 years old subgroup),86.67%(50-60 years old subgroup)and 66.67%(>60 years old subgroup),with statistically significant differences(P<0.05).The△ST/△HR index could improve the sensitivity and specificity of treadmill exercise test.Conclusion Treadmill exercise test has certain value in the diagnosis of male coronary heart disease.As the age increases,the specificity of treadmill exercise test decreases.The degree of ST-segment depression and the degree of increased heart rate(i.e.,△ST/△HR)can improve the sensitivity and specificity of treadmill exercise test,which is of great significance in the diagnosis of male coronary heart disease.
