丹栀逍遥散加减联合丙硫氧嘧啶治疗肝火旺盛型妊娠期甲状腺功能亢进症临床研究

摘　　要：目的:观察丹栀逍遥散加减联合丙硫氧嘧啶治疗肝火旺盛型妊娠期甲状腺功能亢进症(GHT)的临床疗效。方法:选取120例肝火旺盛型GHT患者,按随机数字表法分为对照组和观察组,每组60例。对照组给予丙硫氧嘧啶治疗,观察组给予丹栀逍遥散加减联合丙硫氧嘧啶治疗。比较2组妊娠结局及治疗前后甲状腺激素、糖脂代谢水平。结果:观察组产后出血量少于对照组(P<0.05),新生儿Apgar评分高于对照组(P<0.05)。观察组产褥感染率为5.00%,低于对照组的18.33%(P<0.05);观察组顺产率为80.00%,高于对照组的58.33%(P<0.05)。治疗后,2组促甲状腺激素(TSH)水平均较治疗前升高(P<0.05),观察组TSH水平高于对照组(P<0.05);2组游离甲状腺素(FT4)、游离三碘甲状腺原氨酸(FT3)、空腹血糖(FBG)、空腹胰岛素(FINS)、总胆固醇(TC)、甘油三酯(TG)水平均较治疗前降低(P<0.05),观察组FT4、FT3、FBG、FINS、TC、TG水平均低于对照组(P<0.05)。结论:丹栀逍遥散加减联合丙硫氧嘧啶治疗肝火旺盛型GHT,可改善患者的妊娠结局,有效调节其甲状腺激素、糖脂代谢水平。Objective:To observe the clinical effect of modified Danzhi Xiaoyao powder combined with propylthiouracil for gestational hyperthyroidism(GHT)of intense liver fire type.Methods:A total of 120 cases of patients with GHT of intense liver fire type were selected and divided into the control group and the observation group according to the random number table method,with 60 cases in each group.The control group was treated with propylthiouracil,and the observation group was treated with modified Danzhi Xiaoyao powder combined with propylthiouracil.The levels of thyroid hormone and glucose and lipid metabolism before and after treatment as well as the pregnancy outcomes in the two groups were compared.Results:The amount of postpartum hemorrhage in the observation group was less than that in the control group(P<0.05),and neonatal Apgar score was higher(P<0.05).The puerperal infection rate was 5.00%in the observation group,lower than that of 18.33%in the control group(P<0.05);the vaginal delivery rate was 80.00%in the observation group,higher than that of58.33%in the control group(P<0.05).After treatment,the levels of thyroid stimulating hormone(TSH)in the two groups were increased when compared with those before treatment(P<0.05),and the TSH level in the observation group was higher than that in the control group(P<0.05).The levels of free thyroxine(FT4),free triiodothyronine(FT3),fasting blood glucose(FBG),fasting insulin(FINS),total cholesterol(TC)and triglyceride(TG)in the two groups were decreased when compared with those before treatment(P<0.05);the levels of FT4,FT3,FBG,FINS,TC and TG in the observation group were lower than those in the control group(P<0.05).Conclusion:The therapy of modified Danzhi Xiaoyao powder combined with propylthiouracil for GHT of intense liver fire type can improve the pregnancy outcome and effectively regulate the levels of thyroid hormone and glucose and lipid metabolism.
