血清血管黏附蛋白-1与妊娠期糖尿病的关联分析

摘　　要：目的研究血清血管黏附蛋白-1(VAP-1)与妊娠期糖尿病及糖代谢相关指标(空腹血糖、餐后2h血糖、糖化血红蛋白)的关联性。方法选取2019年1月至2020年1月就诊于南京医科大学附属常州市妇幼保健院的150例妊娠期糖尿病患者作为病例组,同时选取150例健康孕妇作为对照组,收集两组孕妇的一般临床资料和检查结果,分析VAP-1水平对糖代谢指标的关联性,以及其对娠期糖尿病的预测价值。结果病例组的VAP-1水平显著高于对照组(3.42±1.53 vs.2.27±0.86),差异具有统计学意义(t=8.025,P<0.05);VAP-1水平与空腹血糖(r=0.462)、餐后2h血糖(r=0.408)、糖化血红蛋白(r=0.429)水平均呈正相关关系(P<0.001);采用受试者工作特征曲线分析血清VAP-1对GDM的诊断价值,曲线下面积为0.718,95%CI为0.661~0.776,P<0.001。结论 VAP-1水平升高与妊娠期糖尿病、糖代谢相关指标水平密切相关,提示检测VAP-1水平可能有助于妊娠期糖尿病的早期诊断。Objective To analyze associations of serum vascular adhesion protein-1(VAP-1) with gestational diabetes mellitus(GDM) and related indexes of glucose metabolism(fasting blood glucose, 2 h postprandial blood glucose, glycosylated hemoglobin).Methods 150 pregnant women with GDM who were treated in Changzhou Municipal Maternity and Child Health Hospital Affiliated to Nanjing Medical University from January 2019 to January 2020 were selected as stuty subjects and were divided into case group, and other 150 healthy pregnant women in the same period were selected and divided into control group.The general clinical data and examination results of the pregnant women in the two groups were collected, and the correlation of VAP-1 level with glucose metabolism indexes and its predictive value for GDM were analyzed.Results The VAP-1 level of the pregnant women in the case group was significantly higher than that in the control group(3.42±1.53 vs.2.27±0.86),and the difference was statistically significant(t=8.025,P<0.05).VAP-1 level was positively correlated with fasting blood glucose(r=0.462) and 2 h postprandial blood glucose(r=0.408) and glycosylated hemoglobin(r=0.429) levels(all P<0.001).The diagnostic value of serum VAP-1 for GDM was analyzed by receiver operating characteristics(ROC) curve, the results showed that the area under the curve(AUC) was 0.718,95%CI was 0.661~0.776,P<0.001.Conclusion The elevated serum VAP-1 level is closely associated with GDM and the levels of related indexes of glucose metabolism during pregnancy, which suggests that detection of serum VAP-1 level may be helpful for early diagnosis of GDM.
