输卵管峡部妊娠的经阴道超声图像特征分析

摘　　要：目的:分析输卵管峡部妊娠的经阴道超声图像特征。方法:回顾性分析2020年1月~2021年1月本院收治的输卵管峡部妊娠患者的超声表现,共计24例,术前均行经阴道超声检查,均经手术及病理证实为输卵管峡部妊娠。结果:输卵管峡部妊娠的超声表现为:孕囊或包块位于子宫外侧,与子宫紧邻,周边无肌层包绕,探头推压不能与子宫产生相对运动。超声诊断符合率83%(20/24),24例患者中误诊为输卵管间质部妊娠1例,笼统诊断异位妊娠2例,漏诊1例。其中孕囊型5例,输卵管环型17例,混合性包块型2例。结论:经阴道超声对输卵管峡部妊娠具有较高的定位价值,对输卵管峡部妊娠的早期发现和诊断有重大意义。Objective:The transvaginal ultrasound images of isthmus tubal pregnancy were analyzed.Methods:The ultrasonic manifestations of tubal isthmus pregnancy in our hospital from January 2020 to January 2021 were retrospectively analyzed.A total of 24 cases were examined by vaginal ultrasound before operation,all of which were confirmed by operation and pathology as tubal isthmus pregnancy.Results:The ultrasound performance of pregnancy in the tubal isthmus is:the pregnancy sac or package is located on the outside of the uterus,adjacent to the uterus,there is no surrounding muscle coating,and the probe push pressure can not produce relative movement with the uterus.The ultrasonic diagnostic compliance rate is 83%(20/24).Among the 24 patients,1 was misdiagnosed as tubal interstitial pregnancy,2 was ectopic pregnancy and 1 was missed.Among them,5 cases of gestational sac,17 cases of fallopian tube ring and 2 case of mixed mass.Conclusion:Transvaginal ultrasound has high positioning value for tubal isthmus pregnancy and is of great significance for early detection and diagnosis of isthmus pregnancy.
