摘　　要：目的探讨肺肉瘤样癌(pulmonary sarcomatoid carcinoma,PSC)的临床特点、影像学征象,为临床诊疗提供帮助。方法收集2008年2月—2014年3月该院诊治的25例肉瘤样患者的临床资料、病理信息及影像学特征,进行回顾性分析。结果PSC好发于老年男性,常伴有长期重度吸烟史,5年生存率低。病灶多发于右肺,首诊病灶多>5 cm且伴有中央低密度区或空洞,边缘可见渐进性中度强化,与胸膜关系较为密切,易发生纵隔淋巴结转移及远处转移。随访病例生存期为1~45个月,中位生存期为13.7个月,3年、5年生存率9.1%、0.0%极低。结论PSC临床症状缺乏特异性,症状相对隐匿,易发生转移,预后差。影像学特征相对特异,有助于疾病诊断。Objective To explore the clinical characteristics and imaging signs of PSC,and to provide help for clinical diagnosis and treatment.Methods The clinical data,pathological information and imaging characteristics of 25 sarcomatoid patients diagnosed and treated in the hospital from February 2008 to March 2014 were collected and retrospectively analyzed.Results Pulmonary sarcomatoid cancer occurred more often in elderly men,often accompanied by a long-term history of heavy smoking,and the 5-year survival rate was low.The lesions were mostly in the right lung.The first lesions were usually larger than 5 cm and accompanied by a central low-density area or cavity.Progressive and moderate enhancement can be seen at the edge.It was closely related to the pleura and was prone to mediastinal lymph node metastasis and distant metastasis.The shortest survival period of follow-up cases was 1 month,the longest was 45 months,and the average survival period was 13.7 months.The 3-year and 5-year survival rates were 9.1%and 0.0% were extremely low.Conclusion The clinical symptoms of pulmonary sarcomatoid carcinoma lack specificity,the symptoms are relatively insidious,metastasis is prone to occur,and the prognosis is poor.The imaging features are relatively specific,which is helpful for disease diagnosis.
