摘  要:目的观察参附注射液对脓毒症患者外周血T细胞亚群、Treg细胞亚群、NK细胞亚群以及ICU住院时间的影响。方法选取2019年4月-2021年3月该院收治的57例确诊脓毒症患者作为研究对象,利用简单随机法分为对照组(28例)和治疗组(29例)。对照组给予脓毒症常规集束化治疗,治疗组在对照组治疗的基础上加用参附注射液(100mL静脉滴注,1次/d)。抽取静脉血以流式细胞技术测定患者治疗前、治疗后3 d和7 d的外周血T细胞亚群(CD3^(+),CD4^(+),CD8^(+))、Treg细胞亚群(CD3^(+)CD4^(+)CD25hiCD127Low)及NK细胞亚群(CD3^(-)CD56^(+)CD16^(+))的百分比的变化。结果治疗前,两组患者外周血T细胞亚群、NK细胞亚群、Treg细胞亚群比较差异无统计学意义(P>0.05),治疗后两组患者外周血T细胞亚群(CD3^(+),CD4^(+),CD4^(+)/CD8^(+))均较治疗前升高,NK细胞亚群(CD3^(-)CD56^(+)CD16^(+))及Treg细胞亚群(CD3^(+)CD4^(+)CD25hiCD127Low)均有不同程度的降低趋势,尤其7 d后治疗组外周血CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)均较对照组明显升高,差异有统计学意义(P<0.001),Treg细胞亚群在治疗3 d和7 d治疗组较对照组明显降低,差异有统计学意义(P<0.001),而NK细胞亚群两组比较差异无统计学意义(P=0.345),同时两组患者APACHE-Ⅱ和SOFA评分均同步降低,且治疗组较对照组降低更明显,差异有统计学意义(P<0.05)。结论脓毒症的常规治疗联合应用参附注射液,能显著改善患者的炎症因子水平以及免疫抑制,增强免疫储备,改善预后。Objective To observe the effect of Shenfu injection on peripheral blood T cell subsets,Treg cell subsets,NK cell subsets and length of stay in ICU in patients with sepsis.Methods 57 patients with confirmed sepsis treated in the hospital from April 2019 to March 2021 were selected as the research object.They were randomly divided into control group(28 cases)and treatment group(29 cases).The control group was given conventional cluster treatment of sepsis,and the treatment group was based on the treatment of the control group,added Shenfu injection(100 mL intravenous drip,1 times/d).Venous blood was drawn to determine the peripheral blood T cell subsets(CD3^(+),CD4^(+),CD8^(+)),Treg cell subsets(CD3^(+)CD4^(+)CD25hiCD127Low)and NK cell subsets before treatment,3 d and 7 d after treatment by flow cytometry.The change in the percentage of the group(CD3^(-)CD56^(+)CD16^(+)).Results Before treatment,there was no significant difference in peripheral blood T cell subsets,NK cell subsets,and Treg cell subsets between the two groups of patients(P>0.05).After treatment,the two groups of patients had peripheral blood T cell subsets(CD3^(+),CD4^(+),CD4^(+)/CD8^(+))were all higher than before treatment,and NK cell subgroups(CD3^(-)CD56^(+)CD16^(+))and Treg cell subgroups(CD3^(+)CD4^(+)CD25hiCD127Low)had different degrees of decreasing trends,especially in the periphery of the treatment group after 7 days Blood CD3^(+),CD4^(+),CD4^(+)/CD8^(+) were significantly higher than those in the control group,the difference was statistically significant(P<0.001),the Treg cell subsets were significantly lower than the control group in the treatment group for 3 days and 7 days,and the difference was statistically significant(P<0.001),but there was no significant difference between the two groups of NK cell subsets(P=0.345).At the same time,the APACHE-Ⅱand SOFA scores of the two groups of patients decreased simultaneously,and the treatment group was lower than the control group,and the difference was statistically significant(P<0.05).Conc
