七氟烷联合丙泊酚麻醉在妇科手术中的应用及对患者围术期细胞因子平衡的影响

摘　　要：目的探究在妇科手术中运用七氟烷联合丙泊酚麻醉对患者围术期细胞因子平衡产生的影响。方法选取在2017年8月-2019年8月该院进行妇科手术的130患者,随机分为观察组(七氟烷联合丙泊酚麻醉)和对照组(单纯七氟烷麻醉),每组65例,对比两组麻醉效果。结果与对照组比较,T1、T2、T3时观察组血清白细胞介素-8(IL-8)水平均较低,而血清白细胞介素-10(IL-10)水平均较高,差异有统计学意义(t=38.007、20.984、25.008、56.282、8.102、5.311,P<0.05);与对照组相比,观察组苏醒时间、语言恢复时间均较短,并发症发生率(6.15%)较低,差异有统计学意义(t=14.584、14.272;χ^(2)=5.482,P<0.05);术后,观察组认知功能评分(28.89±3.65)分高于对照组(23.14±2.63)分,差异有统计学意义(t=10.304,P<0.05)。结论在妇科手术中运用七氟烷联合丙泊酚麻醉,能有效维持围术期细胞因子的平衡,缩短苏醒时间和语言恢复时间,减少并发症的发生,同时对患者的认知功能具有较小的影响。Objective To explore the effect of sevoflurane combined with propofol anesthesia on the perioperative cytokine balance of patients during gynecological surgery.Methods A total of 130 patients who underwent gynecological surgery in the hospital from August 2017 to August 2019 were randomly divided into observation group(sevoflurane combined with propofol anesthesia)and control group(sevoflurane anesthesia only)with 65 persons each,compared the anesthesia effects of the two groups.Results Compared with the control group,the serum interleukin8(IL-8)level of the observation group was lower at T1,T2,and T3,while the serum interleukin-10(IL-10)level was higher,the difference was statistically significant(t=38.007,20.984,25.008,56.282,8.102,5.311,P<0.05);Compared with the control group,the observation group's wake-up time and language recovery time were shorter,and the complication rate(6.15%)was lower,the difference was statistically significant(t=14.584,14.272;χ^(2)=5.482,P<0.05);The score of cognitive function in the observation group(28.89±3.65)points was higher than that of the control group(23.14±2.63)points,the difference was statistically significant(t=10.304,P<0.05).Conclusion The use of sevoflurane combined with propofol anesthesia in gynecological surgery can effectively maintain the balance of cytokines in the perioperative period,shorten the recovery time and language recovery time,reduce the occurrence of complications,and have a small impact on cognitive function for patients.
