MRT2-mapping成像评估健步虎潜丸联合加味接骨续筋药膏治疗膝关节软骨损伤疗效研究

摘　　要：目的探究MRT2-mapping成像用于评估健步虎潜丸联合加味接骨续筋药膏治疗膝关节软骨损伤疗效的临床价值。方法选择100例膝关节软骨损伤患者,按随机数字表法分为对照组与研究组,各50例。对照组使用常规西药治疗,研究组在对照组基础上使用健步虎潜丸联合加味接骨续筋药膏治疗。2组均治疗4周,治疗前后予MRT2-mapping成像检查,比较中医证候疗效、治疗前后的症状积分[膝关节骨性关节炎严重性指数(ISOA)、纽约特种外科医院膝关节评分(HSS评分)]及关节股骨内/外侧髁、胫骨内/外侧髁4处软骨的弛豫时间(T_(2))值及总平均T_(2)值变化,并通过Pearson积矩相关分析评估膝关节软骨损伤患者的总平均T_(2)值与ISOA、HSS评分间的相关性,探究其临床价值。结果治疗4周后,研究组的总有效率(88.00%,44/50)显著高于对照组(72.00%,36/50)(P<0.05);治疗4周后,2组ISOA较治疗前均显著下降(P<0.05),HSS评分较治疗前均显著上升(P<0.05),且研究组变化幅度更大(P<0.05);治疗4周后,2组4处软骨的T_(2)值及总平均T_(2)值较治疗前均显著下降(P<0.05),且研究组变化幅度更大(P<0.05);经Pearson相关分析,膝关节软骨损伤患者总平均T_(2)值与ISOA具有正相关性(P<0.05),与HSS评分呈负相关性(P<0.05)。结论健步虎潜丸联合加味接骨续筋药膏治疗膝关节软骨损伤疗效显著,可有效改善其膝关节功能,且应用MRT2-mapping成像可有效评估其康复情况。Objective To explore the clinical value of the MRT_(2)-mapping imaging in evaluating the clinical efficacy of the Jianbu Huqian Pills combined with the modified Jiegu Xujin Ointment in the treatment of knee cartilage injuries.Methods A total of 100 patients with knee cartilage injuries were selected as the study subjects and randomly divided into a control group and a study group,with 50 cases in each group.The control group was treated with the conventional western medicine,while the research group was treated with the Jianbu Huqian Pills combined with the modified Jiegu Xujin Ointment on that basis,both for 4 weeks.A MRT_(2)-mapping imaging was performed on the patients before and after treatment,and the curative effect of the TCM syndromes,symptom scores[Index of Severity for Osteoarthritis(ISOA),Hospital for Special Surgery score(HSS score)]and relaxation time(T_(2))value and total mean T_(2) value of cartilage in the femoral joint/lateral condyle,tibial joint/lateral condyle were compared before and after treatment.The correlation between the total mean T_(2) value and ISOA and HSS scores in patients with knee cartilage injuries was evaluated by the Pearson product moment correlation analysis to explore its clinical value.Results After 4 weeks of treatment,the total effective rate of the study group was 88.00%,which was significantly higher than 72.00%of the control group(P<0.05).The ISOA of the two groups were significantly lower than before treatment(P<0.05),while the HSS scores were significantly higher than before treatment(P<0.05),and the change was greater in the study group(P<0.05).After 4 weeks of treatment,the T_(2) values and total mean T_(2) values of 4 cartilages in the 2 groups were significantly decreased compared with those before treatment(P<0.05),and the change was greater in the study group(P<0.05).The Pearson correlation analysis showed that the total average T_(2) value of patients with knee cartilage injuries was positively correlated with the ISOA(P<0.05),and negatively correlated with
