摘　　要：目的观察针灸综合疗法对术后胃轻瘫患者胃液排出量、痞满症状的临床疗效。方法选择30例腹部术后胃轻瘫患者,依照自愿且随机分配原则分成治疗组与对照组,各15例。对照组给予对症支持及西药治疗,治疗组给予针刺中脘、足三里(双侧)、上巨虚(双侧)、内关(双侧)、公孙(双侧)、三阴交(双侧),气海;上巨虚(双侧)、三阴交(双侧)加电针;中脘、足三里(双侧)温针灸,留针30 min,每日1次,以上治疗5 d为1个疗程,连续治疗3个疗程。比较2组胃液引流量、痞满证症状分级(胃部饱胀程度、饱胀持续的时间、压痛、嗳气次数)。结果治疗3个疗程后,治疗组胃液流出量少于对照组(P<0.01);治疗组饱胀程度评分低于对照组(P<0.05);治疗组胃部压痛评分低于对照组(P<0.01);治疗组嗳气评分低于对照组(P<0.05)。结论针灸综合疗法治疗术后胃轻瘫患者,可降低胃液引流量,改善痞满症状。Objective To observe the clinical effect of the comprehensive acupuncture and moxibustion therapy on the gastric juice discharge and distention and fullness symptoms in patients with postoperative gastroparesis.Methods A total of thirty patients with gastroparesis after abdominal surgery were randomly divided into a treatment group and a control group,with 15 cases in each group.The control group was given a symptomatic support and western medicine treatment,while the treatment group was given acupuncture manipulations at Zhongwan(CV12),Zusanli(S36)(bilateral),Shangjuxu(S37)(bilateral),Neiguan(P6)(bilateral),Gongsun(SP4)(bilateral),Sanyinjiao(SP6)(bilateral),Qihai(CV6),and Shangjuxu(S37)(bilateral),Sanyinjiao(SP6)(bilateral)added by electroacupuncture,while Zhongwan(CV12)and Zusanli(S36)(bilateral)added by warm acupuncture,retaining the needles for 30 minutes,once a day,5 days as a treatment course,continuous treatment for 3 courses.The gastric juice discharge and the symptom grading of the distention and fullness syndrome(degree of fullness in stomach,duration of fullness,tenderness and frequency of belching)were compared between the two groups.Results After 3 courses of treatment,the gastric juice outflow of the treatment group was less than that of the control group(P<0.01).The fullness score of the treatment group was lower than that of the control group(P<0.05).The gastric tenderness score of the treatment group was lower than that of the control group(P<0.01).The belching score of the treatment group was lower than that of the control group(P<0.05).Conclusion The comprehensive acupuncture and moxibustion therapy can reduce the gastric juice drainage and improve symptoms of distention and fullness in patients with postoperative gastroparesis.
