关节镜下半月板部分切除术在膝关节半月板损伤患者中的应用研究

摘　　要：目的探讨关节镜下半月板部分切除术在膝关节半月板损伤患者中的应用效果。方法选择2018年6月—2020年5月该院收治的膝关节半月板损伤患者98例,以随机数字表将其划分为两组。对照组49例行关节镜下半月板成形术,研究组49例行关节镜下半月板部分切除术。比较两组患者的临床疗效,治疗前与治疗1年时的膝关节活动度、膝关节疼痛与功能情况,以及术后并发症情况。结果研究组治疗的优良率93.88%较对照组77.55%高,差异有统计学意义(χ^(2)=5.333,P<0.05)。治疗1年时,研究组膝关节活动度(132.52±12.72)°、Lysholms评分(90.65±8.52)分较对照组(125.03±11.45)°、(84.03±6.06)分高,差异有统计学意义(t=3.064,3.726,P<0.05),VAS量表评分(0.98±0.20)分较对照组(1.26±0.23)分低,差异有统计学意义(t=6.431,P<0.05)。研究组术后并发症发生率2.04%与对照组6.12%对比差异无统计学意义(χ^(2)=0.261,P>0.05)。结论关节镜下半月板部分切除术能够有效改善膝关节半月板损伤患者的疼痛症状,促进关节功能恢复,适于临床应用。Objective To investigate the effect of arthroscopic partial meniscectomy in patients with knee meniscus injury.Methods A total of 98 patients with knee meniscus injury admitted to this hospital from June 2018 to May 2020 were selected and divided into two groups using a random number table.In the control group,49 cases underwent arthroscopic meniscus plasty,and 49 cases in the study group underwent arthroscopic partial meniscus resection.The clinical efficacy of the two groups of patients were compared,the range of motion,knee joint pain and function,and postoperative complications before treatment and 1 year after treatment.Results The excellent and good rate of treatment in the study group was 93.88%higher than that in the control group 77.55%,the difference was statistically significant(χ^(2)=5.333,P<0.05).At 1 year of treatment,the knee joint range of motion(132.52±12.72)°and Lysholms score(90.65±8.52)points in the study group were higher than those in the control group(125.03±11.45)°and(84.03±6.06),and the difference was statistically significant(t=3.064,3.726,P<0.05),the score of the VAS scale(0.98±0.20)points was lower than that of the control group(1.26±0.23)points,and the difference was statistically significant(t=6.431,P<0.05).There was no significant difference in the postoperative complication rate of 2.04%in the study group and 6.12%in the control group(χ^(2)=0.261,P>0.05).Conclusion Arthroscopic partial meniscectomy can effectively improve the pain symptoms and joint function of patients with knee meniscus injury,which is suitable for clinical application and promotion.
