比较Wiltse入路与传统后正中入路TLIF手术在治疗单节段腰椎退变性滑脱的疗效

摘　　要：目的比较Wiltse入路手术与传统后正中入路TLIF手术治疗单节段腰椎退变性滑脱的治疗效果。方法选取该院2017年1月—2020年10月收治的单节段退变性腰椎滑脱病例105例。将患者按照治疗方式的差异性分为治疗组53例,对照组52例。给予对照组患者正中入路TLIF治疗,给予治疗组Wiltse入路TLIF治疗。对比两组患者的手术指标、肌酸磷酸激酶(CPK)水平、视觉模拟评分(VAS评分)及Oswestry功能障碍指数(ODI评分)。结果治疗组患者手术时间(125.66±10.36)min、术中出血量(218.35±23.22)mL、术后引流量(146.99±25.66)mL、住院时间(6.55±1.05)d,均低于对照组手术时间(156.34±10.52)min、术中出血量(398.55±12.11)mL、术后引流量(264.67±22.36)mL、住院时间(9.33±1.28)d,组间手术指标差异有统计学意义(t=15.056、49.717、25.034、12.177,P<0.05);治疗组与对照组患者术后CPK水平变化明显,差异有统计学意义(P<0.05);术后治疗组患者VAS及ODI评分显著小于对照组,差异有统计学意义(P<0.05)。结论Wiltse入路TLIF术式治疗单节段退变性腰椎滑脱的疗效显著,各项指标良好,治疗意义重大。Objective To compare the therapeutic effects of Wiltse approach and traditional posterior median approach TLIF in the treatment of single segment lumbar degenerative spondylolisthesis.Methods A total of 105 cases of single-segment degenerative lumbar spondylolisthesis admitted to the hospital from January 2017 to October 2020 were selected.Divided the patients into a treatment group of 53 cases and a control group of 52 cases according to the differences in treatment methods.The study gave the control group patients the median approach TLIF treatment,and the treatment group gave the Wiltse approach TLIF treatment.The surgical indexes,creatine phosphokinase(CPK)level,visual analog scale(VAS score)and Oswestry dysfunction index(ODI score)of the two groups were compared.Results In the treatment group,the operative time was(125.66±10.36)min,the intraoperative blood loss was(218.35±23.22)mL,the postoperative drainage volume was(146.99±25.66)mL,the hospital stay was(6.55±1.05)d,compared with the control group(156.34±10.52)min,intraoperative blood loss(398.55±12.11)mL,postoperative drainage flow(264.67±22.36)mL,hospital stay(9.33±1.28)d,there were statistically significant differences in surgical indexes between groups,the difference was statistically significant(t=15.056,49.717,25.034,12.177,P<0.05).The level of CPK in the treatment group and the control group changed significantly after surgery,and the difference was statistically significant(P<0.05).After operation,VAS and ODI scores in treatment group were significantly lower than those in control group,the difference was statistically significant(P<0.05).Conclusion The curative effect of Wiltse approach TLIF in the treatment of single segment degenerative lumbar spondylolisthesis is significant,all indexes are good,the treatment is of great significance.
