二维及三维超声检查技术在产前胎儿唇腭裂畸形筛查中的应用价值

摘　　要：目的于产前胎儿唇腭裂筛查中联合应用二维及三维超声检查技术,通过辨别声像图表现特征,提高产前唇腭裂的诊断符合率。方法应用二维及三维超声检查技术对2017年1月至2019年12月在本院进行产前超声筛查的孕妇进行产前超声检查,对7023例胎儿超声声像图特征进行调查,总结正常胎儿及唇腭裂胎儿各自的超声声像图特征,并与临床引产或顺产后进行对比,分析该检查技术的诊断符合率。结果在7023例胎儿中,通过超声检查产前共检出唇腭裂15例,其中单侧上唇裂9例,双侧上唇裂2例,双侧唇腭裂2例,单纯Ⅱ度腭裂1例,唇裂并牙槽突裂1例。经引产或顺产后进行对比,漏诊1例,诊断符合率达99.98%(7022/7023)。结论孕妇产前行超声二维结合三维的多切面筛查可以较准确观察胎儿唇腭裂的情况,对临床制定治疗策略、评估预后有重要意义。Objective:To improve the diagnostic consistency rate of prenatal cleft lip and palate by identifying the characteristics of the ultrasonography by using two-dimensional and three-dimensional ultrasonography in prenatal cleft lip and palate screening.Methods:A total of 7023 fetuses admitted by our hospital from January 2017 to December 2019 were examined by 2d and 3D ultrasonography.The ultrasonographic characteristics of normal fetuses and fetuses with cleft lip and palate were summarized,and the diagnostic coincidence rate of the ultrasonography was analyzed by comparing with the clinical induced labor or vaginal labor.Results:Among 7023 fetuses,15 cases of cleft lip and palate were detected by ultrasonic examination before delivery,including 9 cases of unilateral upper lip,2 cases of bilateral upper lip and palate,2 cases of bilateral cleft lip and palate,1 case of cleft lip and alveolar cleft.After induced labor or vaginal labor,1 case was missed,and the diagnosis coincidence rate was 93.75%.Conclusion:Prenatal ultrasound combined with three-dimensional multi-section screening can accurately observe the situation of fetal cleft lip and palate,which is of great significance for clinical treatment strategy and prognosis assessment.
