摘　　要：目的探讨甲状腺功能亢进症合并骨质疏松的患者自身免疫指标与细胞炎症因子水平的相关性。方法选取初诊甲状腺功能亢进症患者132例,根据骨密度含量分为骨质疏松组(48例)、骨量减少组(44例)和骨量正常组(40例);选取同期门诊健康体检者42例为对照组。测定各组T淋巴细胞亚群(CD3^(+)、CD4^(+)、CD8^(+))、白细胞介素-6(IL-6)、IL-18及肿瘤坏死因子-α(TNF-α)水平,采用Spearman相关性分析CD3^(+)、CD4^(+)、CD8^(+)与IL-6、IL-18、TNF-α的相关性。结果骨量正常组、骨量减少组、骨质疏松组CD3^(+)、CD4^(+)、CD8^(+)水平低于对照组(P<0.05),IL-6、IL-18、TNF-α水平高于对照组(P<0.05)。Spearman相关性分析显示,甲状腺功能亢进症合并骨质疏松的患者CD3^(+)、CD4^(+)、CD8^(+)水平与血清IL-6、IL-18、TNF-α水平均呈正相关(P<0.05)。结论甲状腺功能亢进症合并骨质疏松的患者存在明显的免疫失调和炎症反应,其自身免疫指标与细胞炎症因子水平呈正相关。Aim To investigate the correlation between autoimmune indexes and cytokine levels in patients with hyperthyroidism complicated with osteoporosis.Methods 132 newly diagnosed patients with hyperthyroidism were divided into osteoporosis group(48 cases),bone mass reduction group(44 cases)and normal bone mass group(40 cases);42 healthy outpatients in the same period were selected as the control group.The levels of T lymphocyte subsets(CD3^(+),CD4^(+),CD8^(+)),interleukin-6(IL-6),IL-18 and tumor necrosis factor were measured.Spearman correlation analysis was used to analyze the relationship between CD3^(+),CD4^(+),CD8^(+)and IL-6,IL-18 and TNF-αrelevance.Results The levels of CD3^(+),CD4^(+),CD8^(+)in normal bone mass group,osteopenia group and osteoporosis group were lower than those in the control group(P<0.05).The level of IL-6,IL-18 and TNF-αwas higher than that in the control group(P<0.05).Spearman correlation analysis showed that the levels of CD3^(+),CD4^(+),CD8^(+)and serum IL-6,IL-18 and TNF in patients with hyperthyroidism complicated with osteoporosisαlevels were positively correlated(P<0.05).Conclusion Patients with hyperthyroidism complicated with osteoporosis have obvious immune imbalance and inflammatory reaction,and the level of autoimmune indexes is positively correlated with the level of cytokines.
