摘 要:目的探讨瞬感扫描式葡萄糖监测(flash glucose monitoring,FGM)技术在胰岛素强化治疗2型糖尿病患者中的应用效果。方法选择2019年4—12月本院收治的2型糖尿病患者40例作为研究对象,按照随机数表法分为对照组和观察组两组,每组各20例。患者均采用谷赖胰岛素+甘精胰岛素强化治疗,对照组使用罗氏血糖仪监测血糖,观察组使用瞬感扫描式葡萄糖监测(FGM)技术监测血糖,比较两组患者治疗后临床效果、低血糖发生次数和满意度、最大血糖波动幅度(largest amplitude of glycemic excursions,LAGE)、平均血糖波动幅度(mean amplitude of glycemic excursions,MAGE)、血糖水平标准差(standard deviation of blood glucose,SDBG)和达标范围内时间(time in range,TIR)等血糖变异性指标。结果与治疗前比,两组空腹血糖、餐后2 h血糖显著降低(P<0.001),组间比较差异无统计学意义(P>0.05)。观察组低血糖发生次数、LAGE、MAGE和SDBG明显低于对照组(P<0.05),TIR和满意度高于对照组(P<0.05)。结论瞬感扫描式葡萄糖监测(FGM)技术能够减少胰岛素强化治疗2型糖尿病患者的血糖波动、低血糖发生并提升患者满意度。Objective To investigate the application effect of flash glucose monitoring(FGM)technique on intensive insulin treatment of patients with type 2 diabetes.Methods 40 patients with type 2 diabetes those were admitted to Yijishan hospital of Wannan Medical College from April to December 2019were selected as study objects,they were divided into control group and study group according to random number table method,20 cases in each group.All the objects were treated with intensive insulin treatment ofglulisine and glargine.The control group used Roche blood glucose meter to monitor glucose,while the study group used FreeStyle Libre Flash Glucose Monitoring System to monitor glucose.The clinical efficacy,the frequency of hypoglycemia,treatment satisfaction and blood glucose variability(including the largest amplitude of glycemic excursions,LAGE;mean amplitude of glycemic excursions,MAGE;standard deviation of blood glucose,SDBG;time in range,TIR)were compared between the two groups.Results Fasting plasma glucose(FPG)and 2-hour postprandial blood glucose(2hPG)were significantly down-regulated in patients in both groups following insulin administration(P<0.001),however,there was no significant difference between the two groups(P<0.05).The frequency of hypoglycemia,LAGE,MAGE and SDBG in the study group were lower than those in the control group(P<0.05),TIR and the patient satisfaction was higher than those in the control group(P<0.05).Conclusions FGM could reduce blood glucose fluctuation,hypoglycemia and improve patient satisfaction in patients with type 2 diabetes after intensive insulin treatment.
