两种复合麻醉对妇科腹腔镜手术患者血流动力学及脑氧饱和度的影响

摘　　要：目的探究七氟醚复合麻醉、异丙酚复合麻醉对妇科腹腔镜手术患者血流动力学及脑氧饱和度的影响,为临床麻醉医师选择合理麻醉方案提供参考。方法选取2019年1—12月于本院接受妇科腹腔镜手术的80例患者作为研究对象,按照随机数字表法分为A组与B组,每组40例。A组采用七氟醚复合麻醉,B组采用异丙酚复合麻醉,比较两组血流动力学指标、脑氧饱和度。结果A组插管即刻、插管后5 min、切皮时、气腹5 min呼吸末二氧化碳分压水平均低于B组(P<0.05);A组插管即刻、插管后5 min、切皮时、气腹5 min平均动脉压高于B组(P<0.05);术前、麻醉5 min后、插管即刻、插管后5 min、气腹时、放气腹时、拔管5 min后,A组脑氧饱和度均高于B组(P<0.05)。结论在妇科腹腔镜手术过程中,七氟醚复合麻醉与异丙酚复合麻醉均具有显著的临床麻醉效果,但七氟醚复合麻醉相较于异丙酚复合麻醉,患者血流动力学更稳定,脑氧饱和度更好,值得临床推广应用。Objective To explore the effects of combined sevoflurane anesthesia and propofol combined anesthesia on hemodynamics and cerebral oxygen saturation in patients undergoing gynecological laparoscopic surgery,and to provide reference for clinical anesthesiologists to choose reasonable anesthesia plan.Methods 80 patients who underwent gynecological laparoscopic surgery in our hospital from January to December 2019 were selected as the research subjects,and they were divided into group A and group B according to the random number table method,with 40 cases in each group.Group A was treated with sevoflurane combined anesthesia,and group B was treated with propofol combined anesthesia.The hemodynamic indexes and cerebral oxygen saturation between the two groups were compared.Results The partial pressure of carbon dioxide at the end of breath of group A were lower than group B immediately after intubation,5 min after intubation,skin incision,and 5 min after pneumoperitoneum(P<0.05);the average arterial pressure of group A immediately after intubation,5 min after intubation,skin incision and 5 min after pneumoperitoneum was higher than that of group B(P<0.05);before operation,5 min after anesthesia,immediately after intubation,5 min after intubation,during pneumoperitoneum,during deflation,and extubation after 5 min,the cerebral oxygen saturation of group A were higher than group B(P<0.05).Conclusion In the process of gynecological laparoscopic surgery,sevoflurane combined anesthesia and propofol combined anesthesia have significant clinical anesthesia effect.However,compared with propofol combined anesthesia,sevoflurane combined anesthesia has more stable hemodynamics and better cerebral oxygen saturation,which is worthy of clinical promotion.
