摘  要:目的探索针刺三阴交得气对寒湿凝滞型原发性痛经患者能量代谢指标的影响。方法采用随机对照试验,纳入寒湿凝滞证原发性痛经患者8例,分为得气组和不得气组各4例,2组均针刺双侧三阴交穴,得气组给予粗针、深刺、行手法的促使得气干预,不得气组给予细针、浅刺、不行手法的避免得气干预,应用受试者针感临床评价量表测评实际得气情况。同时记录2组针刺前、中、后共12个时点的红外热像温度、微循环血流灌注量,观察三阴交穴得气与否两种状态下指标是否存在差异。结果通过试验发现三阴交得气与否不同状态下,公孙穴区微循环血流灌注量、三阴交穴区和阴陵泉-血海段经脉的红外热像温度等指标存在差异(P<0.05)。结论在留针期间或起针后一些时点,三阴交得气时公孙微循环血流灌注量较不得气时更充沛,三阴交穴区和阴陵泉-血海段经脉的红外热像温度较不得气时更高,说明能量代谢指标可能客观反映三阴交得气与否的不同状态。Objective: To explore the effect of Deqi induced by needling Sanyinjiao(SP6) on energy metabolism indicators in patients with primary dysmenorrhea in cold and dampness stagnation pattern. Methods: A randomized controlled trial was carried out. 8 patients with primary dysmenorrhea in cold and dampness stagnation pattern were enrolled and allocated to deqi group and non deqi group, with 4 cases in each group. Both groups accepted acupuncture in left and right Sanyinjiao(SP6). deqi group accepted needle insertion with thick needle, deep insertion and manipulation for deqi. non deqi group accepted needle insertion with thin needle, shallow insertion and no manipulation without deqi. Deqi of the subjects was assessed using the score scale. Energy metabolism indicators, which were infrared thermal image and microcirculation blood flow perfusion volume, were recorded before, during and after acupuncture. The effects of deqi and non-deqi at acupoint Sanyinjiao(SP6) on the indicators were observed. Results: The trial test showed that, under the different states of deqi or non-deqi at Sanyinjiao(SP6), the microcirculation blood flow perfusion volume of Gongsun, the infrared thermal image temperature of Sanyinjiao and Yinlingquan-Xuehai meridian segment are different(P<0.05).Conclusion: During some time points which are in the acupuncture period or after, the microcirculation blood flow perfusion volume of Gongsun is more abundant when deqi at Sanyinjiao, and the infrared thermal image temperature of Sanyinjiao and YinlingquanXuehai meridian segment is higher than when deqi at Sanyinjiao, indicating that the energy metabolism indicators may objectively reflect the different states of deqi or non-deqi.
