米库氯胺在腹腔镜全子宫切除术中维持深度肌松的应用价值

摘　　要：目的:探讨米库氯铵在腹腔镜全子宫切除术中维持深度肌松的应用价值。方法:选择福建省妇幼保健院2020年6月至2021年1月择期行腹腔镜全子宫切除术患者60例,采用随机数字表法分为米库氯铵组(M组)和罗库溴铵组(R组),每组30例,比较两组患者的肌松时效指标,血流动力学指标等。结果:M组患者的各个肌松监测指标均明显短于R组,差异均具有统计学意义(P<0.05);拔管时,M组患者的肌松残余率显著低于R组,差异具有统计学意义(P<0.05);两组患者麻醉诱导前、气管插管时、手术结束时、气管拔管时、拔管后30 min各时间点的心率(HR)及平均动脉压(MAP)比较,差异均无统计学意义(P>0.05)。结论:腹腔镜全子宫切除术时使用米库氯铵维持深度肌松,可以取得较好的效果,且术后恢复快,肌松残余率低。Objective To explore the application value of micuronium in maintaining deep muscle relaxation during laparoscopic hysterectomy.Methods A total of 60 patients undergoing elective laparoscopic hysterectomy in Fujian Maternity and Child Health Hospital from June 2020 to January 2021 were selected and divided into micuronium group(M group)and rocuronium group by random number table.Group(R group),30 cases in each group,compare the time-dependent indexes of muscle relaxation and hemodynamic indexes of the two groups of patients.Results The muscle relaxation monitoring indexes of the M group were significantly shorter than those of the R group,and the difference was statistically significant(P<0.05);when the tube was extubated,the residual rate of muscle relaxation of the M group was significantly lower than that of the R group,and the difference was statistically significant(P<0.05);there was no statistically significant difference in HR and MAP between the two groups of patients before induction of anesthesia,at the time of tracheal intubation,at the end of the operation,at the time of tracheal extubation,and 30 min after extubation(P>0.05).Conclusion The use of micuronium to maintain deep muscle relaxation during laparoscopic total hysterectomy can achieve better results,and the postoperative recovery is fast,and the residual muscle relaxation rate is low.
