封闭式负压引流联合游离植皮术对深度烧伤感染创面的效果研究

摘　　要：目的:探讨封闭式负压引流技术联合游离植皮对深度烧伤感染创面的效果。方法:选取2017年2月至2020年2月遵义医科大学第五附属(珠海)医院收治的深度烧伤感染创面患者76例,随机分为对照组和观察组,各38例。对照组给予常规游离植皮治疗,观察组给予封闭式负压引流技术联合游离植皮治疗。比较两组清创至终末手术时间、创面愈合时间、疼痛评分、换药次数、治疗优良率、安全性。结果:观察组患者清创至终末手术时间、创面愈合时间均短于对照组,疼痛评分低于对照组,换药次数少于对照组,差异均具有统计学意义(P<0.05);观察组患者治疗优良率高于对照组,差异具有统计学意义(P<0.05);观察组患者的感染、积液、弹力性水疱、出血、二次缝合发生率均低于对照组,差异具有统计学意义(P<0.05)。结论:深度烧伤感染创面患者采用封闭式负压引流技术联合游离植皮治疗,患者创面愈合速度加快,减轻患者疼痛感,护理方便且安全性较高。Objective To investigate the effect of closed negative pressure drainage combined with free skin grafting on deep burn infected wounds.Methods A total of 76 patients with deep burn infection wounds admitted to the Fifth Affiliated(Zhuhai)Hospital of Zunyi Medical University from February 2017 to February 2020 were randomly divided into two groups,38 cases in the control group and 38 cases in the observation group.The control group was treated with conventional free skin grafting,and the observation group was treated with closed negative pressure drainage combined with free skin grafting.The time from debridement to terminal operation,wound healing time,pain score,dressing change times,excellent and good rate of treatment and safety were compared between the two groups.Results The time from debridement to terminal surgery and wound healing time were shorter in the observation group than in the control group,the pain score was lower than that in the control group,and the frequency of dressing change was less than that in the control group,with statistical significance(P<0.05).The excellent and good rate of observation group was higher than that of control group,and the difference was statistically significant(P<0.05).The incidence of infection,effusion,elastic blister,bleeding and secondary suture in observation group was lower than that in control group,and the difference was statistically significant(P<0.05).Conclusion Patients with deep burn infected wounds are treated with closed negative pressure drainage technology combined with free skin grafting,which can accelerate the healing speed of patients,reduce the pain of patients,convenient nursing,high safety,and obvious therapeutic effect.
