摘 要:目的查清中山市一起布鲁菌病(布病)疫情的感染来源和传播危险因素。方法对病例、共同暴露者、暴露羊只开展流行病学调查和布鲁菌相关项目检测,采用描述性流行病学方法对疫情资料进行分析。结果本次疫情报告2例确诊病例,为母子关系,年龄分别为69岁和37岁。2例病例分别于1月、5月发病,主要临床表现不同,病例1主要为骨关节和造血系统受损,突出症状为腰痛、贫血;病例2反复出现发热、畏寒、多汗。2例病例均有7年的山羊养殖史,经常在无防护措施下接触山羊及其产物。病例血清学检测结果为1∶200,布鲁菌型别鉴定为羊种3型。病家6名共同暴露者血清学检测结果为阴性。病家养殖的羊只血清标本检出阳性。结论本次疫情由羊种3型布鲁菌引起,频繁直接接触病羊及其排泄物、阴道分泌物、流产羊胎盘是疫情发生的危险因素。为减少类似事件发生,应充分发挥联防联控机制,提高源头防控,加强重点场所的检疫、监管和高风险暴露人群的健康教育。Objective To investigate the infection source and transmission risk factors of brucellosis in Zhongshan.Methods Epidemiological investigation and related laboratory tests of brucellosis were conducted on cases, co-exposers and exposed goats, descriptive epidemiological method was used to analyze the epidemic situation data.Results Two confirmed cases with mother-child relations were reported in the epidemic, the age of the cases were 69 and 37 years old respectively.The time of disease onset was January and May respectively, primary clinical manifestations varied.Case 1 was clinically manifested as osteoarthropathy and damage of hematopoietic system, and primarily manifested as lumbago and anemia, case 2 showed fever, chill, repeated sweats.Both cases had the history of goat breeding for 7 years, were in frequent contact with goats and corresponding products at unprotected measures.The outcome of serum samples of cases with serological test was 1∶200,the bacterial type of brucellosis was type 3 brucella melitensis.Serum samples from all 6 co-exposed individuals were negative by serological tests, and the serum samples of exposed goats bred in sickened cases were positive.Conclusion The epidemic is caused by type 3 brucella melitensis. Frequent direct contact with sickened goats and their excreta, vaginal secretions, aborted goat placentas are considered to be the risk factors of the epidemic.To reduce the occurrence of similar incidents, the joint prevention and control mechanism should be developed adequately, the source prevention and control should be improved, the quarantine and supervision should be tightened in key places, and public health education for individuals with high-risk exposures should be improved.
