摘　　要：目的调查某三级医院抗菌药物使用情况和变化趋势,并分析和评价其抗菌药物临床应用管理的长期效果。方法利用医院信息系统收集某三级医院2012年1月至2019年12月全身用抗菌药物使用相关数据,参考抗菌药物用药指标,对数据进行统计和分析。结果虽然某院2014年后抗菌药物临床应用整体情况较2012年明显改善,但2014年之后变化趋缓。2019年,医院住院患者抗菌药物使用率、使用强度和门诊处方抗菌药物使用率分别为58.63%、51.47 DDDs/(100人·d)、24.28%。结论与国内其他研究相比,某院抗菌药物临床应用情况未实现明显的持续改善作用。当前,需要完善医院抗菌药物评价方法,制定院内指南及开展处方前置审核工作,并进一步加强临床应用管理。Objective To investigate the use and change trend of antimicrobial drug in a tertiary hospital,and to analyze and evaluate the long-term effect of clinical application management of antimicrobial drug.Methods The hospital information system was used to collect the data related to the use of systemic antimicrobial drug in a tertiary hospital from January 2012 to December 2019,and the data were counted and analyzed with reference to the drug use indicators of antimicrobial drug.Results Although the overall situation of clinical application of antimicrobial drug in a hospital after 2014 significantly improved compared with that in 2012,the change slowed down after 2014.In 2019,the utilization rate and use intensity of antibiotics in inpatients and utilization rate of outpatient prescription antibiotics were 58.63%,51.47 DDDs/(100 person·d)and 24.28%respectively.Conclusion Compared with other studies in the country,the clinical application of antimicrobial drug in the hospital did not achieve significant continuous improvement.At present,it is necessary to improve the evaluation method of antimicrobial drug in hospitals,formulate internal guidelines and perform pre-prescription review with feedback,and further strengthen its clinical application management.
