摘  要:目的探讨医学三维可视化系统(IQQA^(®)-3D Liver肝脏评估和治疗计划辅助系统)指导肝脏肿瘤精准切除的应用与围术期护理价值。方法收集43例肝脏切除术患者术前CT或MRI增强检查结果,导入IQQA^(®)-3D Liver系统进行模拟手术,计算出模拟切除肝脏体积,与实际切除肝脏体积进行比较,同时借助三维可视化治疗计划辅助系统来对围术期护理效果进行观察。结果43例患者模拟切除肝脏体积为(390.08±441.52)cm3,实际切除肝脏体积为(408.54±469.32)cm3,其比较差别没有统计学意义(t=0.297,P=0.693);模拟与实际切除肝脏体积之间有显著的正相关性(r=0.968,P<0.01)。且对术后的康复护理辅助显著。结论IQQA^(®)-3D肝脏评估及辅助系统治疗方案可对病变、肝脏实质、肝静脉系统进行三维划分,重建单个肝脏模型,展示了病变与肝内静脉系统的空间关系,并可通过模拟手术功能进行肝脏筛查,计算出肝脏各部位的体积,对指导准确切除肝脏病灶瘤块具有重要意义,同时系统的术前评估和术后辅助护理能够加快患者胃肠道功能恢复,缩短住院时间,有一定的指导价值。Objective To explore the application and perioperative nursing value of 3D medical visualization system(IQQA^(®)-3D Liver assessment and treatment planning assistance system)in guiding precise resection of liver tumors.Methods The results of preoperative CT or MRI enhancement examinations were collected from 43 hepatectomy.The IQQA^(®)-3D Liver system was used for simulated surgery,and the simulated liver volume was calculated and compared with the actual liver volume.In the meantime,the perioperative care was observed with the aid of 3D visualization treatment planning assistance system technology.Results 43 patients had a simulated resected liver volume of(390.08±441.52)cm3,and the actual resected liver volume was(408.54±469.32)cm3.The comparative difference was not statistically significant(t=0.297,P=0.693).There was a significant positive correlation between the simulated and actual resected liver volumes with r=0.968,P<0.01.And it was significantly beneficial for postoperative rehabilitation care.Conclusion The IQQA^(®)-3D Liver assessment and treatment planning assistance system allows 3D classification of lesions,liver parenchyma,and hepatic venous system,and reconstruction a single liver model.The spatial relationship between the lesion and the hepatic venous system is demonstrated,and liver screening can be carried out by simulating the operation function,and the volume of each part of the liver can be calculated.It has great significance to guide the accurate resection of liver lesions and tumor masses.Meanwhile,the system can accelerate the recovery of gastrointestinal function and shorten the length of hospital stay,which has certain guiding value.
