摘 要:目的研究糖化血红蛋白与2型糖尿病周围神经病变发生及疾病进展的关系。方法80例2型糖尿病患者,根据是否发生周围神经病变分为病变组和正常组,每组40例。两组患者均接受糖化血红蛋白、空腹血糖与餐后2 h血糖检查。对比两组的血糖相关指标、病变组不同病变程度患者的血糖相关指标,分析糖化血红蛋白控制情况与周围神经病变的关系。结果病变组患者的糖化血红蛋白(7.9±1.7)%、空腹血糖(7.5±1.9)mmol/L与餐后2 h血糖(15.1±3.7)mmol/L均高于正常组的(6.5±0.7)%、(6.1±0.9)mmol/L、(13.4±2.0)mmol/L,差异具有统计学意义(P<0.05)。病变组感觉+运动纤维异常患者糖化血红蛋白、空腹血糖与餐后2 h血糖均高于单纯运动纤维异常及单纯感觉纤维异常患者,差异具有统计学意义(P<0.05)。病变组中糖化血红蛋白控制达标27例,控制不达标13例。糖化血红蛋白控制达标患者的复合病变率7.4%低于控制不达标患者的69.2%,差异有统计学意义(P<0.05)。结论2型糖尿病的诊断检查中,通过分析糖化血红蛋白相关指标与周围神经病变的关联,发现糖化血红蛋白越高,周围神经病变的程度越严重。Objective To study the correlation between glycosylated hemoglobin and type 2 diabetic peripheral neuropathy.Methods A total of 80 patients with type 2 diabetic peripheral neuropathy were divided into the lesion group and the normal group according to whether peripheral neuropathy occurred or not,with 40 cases in each group.Both groups received tests on glycosylated hemoglobin,fasting blood glucose and 2 h postprandial blood glucose.Comparison was made on the blood glucose-related indicators of the two groups,and the blood glucose-related indicators of patients with different disease severity in the lesion group.The correlation between the control of glycosylated hemoglobin and peripheral neuropathy was analyzed.Results The glycosylated hemoglobin(7.9±1.7)%,fasting blood glucose(7.5±1.9)mmol/L and 2 h postprandial Wood glucose(15.1±3.7)mmol/L in the lesion group were higher than(6.5±0.7)%,(6.1±0.9)mmol/L and(13.4±2.0)mmol/L in the normal group,and the differences were statistically significant(P<0.05).In the lesion group,patients with sensory and motor fiber abnormalities had higher glycated hemoglobin,fasting glucose and 2 h postprandial glucose than those with motor fiber abnormalities alone and sensory fiber abnormalities alone,and the difference was statistically significant(P<0.05).In the lesion group,glycosylated hemoglobin control reached the standard in 27 cases and failed to reach the standard in 13 cases.The composite lesion rate 7.4%in patients with controlled glycosylated hemoglobin was significantly lower than 69.2%in patients without controlled glycosylated hemoglobin,and the difference was statistically significant(P<0.05).Conclusion In the diagnosis of type 2 diabetes mellitus,the correlation between glycosylated hemoglobin related indicators and peripheral neuropathy is analyzed,and it is found that the higher the glycosylated hemoglobin,the more severe the degree of peripheral neuropathy.
