腹部彩超联合胃镜早期诊断腹型过敏性紫癜的临床应用分析

摘　　要：目的:探索胃镜检测方法和腹部彩超检测方法相结合使用于腹型过敏性紫癜临床早期诊断中的价值。方法:对2018年1月~2020年3月本医院诊断的30例腹型过敏性紫癜患儿予以这次对应指标统计,全部患儿都实施胃镜检测、腹部彩超检测,部分患儿接受病理活检,分析胃镜检测方法、腹部彩超检测方法、胃镜相结合腹部彩超检测方法对腹型过敏性紫癜的诊断结果,总结胃镜检测结果表现情况、病理活检结果表现情况、腹部彩超检测结果表现情况。结果:胃镜相结合腹部彩超检测方法对腹型过敏性紫癜的检出不正常合计占比情况对比胃镜检测方法、腹部彩超检测方法均增加(P<0.05),30例患儿病变共46处,十二指肠病变中,22处为糜烂、缺乏规则性表浅溃疡,共3处为充血、水肿表现;十二指肠球病变中,共4处为糜烂、缺乏规则性表浅溃疡,共9处为充血表现、水肿、点形、斑形、条形糜烂出血;胃部病变当中,胃窦、胃体位置会产生黏膜表层点形出血共7处,共1处为融合片形出血病灶。结论:在腹型过敏性紫癜临床早期诊断中选用胃镜检测方法相结合腹部彩超检测方法显示较好诊断效果,两者联合检查可促进早期协诊腹型过敏性紫癜,并为医生治疗提供指导。Objective:To explore the value of gastroscopy combined with color Doppler ultrasonography in the early diagnosis of abdominal anaphylactoid purpura.Methods:From January 2018 to March 2020,30 cases of abdominal anaphylactoid purpura diagnosed by our hospital were included in the corresponding index statistics.All children underwent gastroscopy and abdominal color Doppler ultrasound,and some children received pathological biopsy.Analyze the diagnosis results of abdominal allergic purpura by gastroscopy detection methods,abdominal color Doppler ultrasound detection methods,and gastroscopy combined with abdominal color Doppler ultrasound detection methods,and summarize the performance of gastroscopy detection results,pathological biopsy results,and abdominal color Doppler ultrasound detection results.Results:The proportion of abnormal total detected by gastroscopy combined with abdominal color Doppler ultrasonography was higher than that by gastroscopy and abdominal color Doppler ultrasonography(P<0.05),30 Children had 46 lesions,of duodenal lesions,22 were erosion and lack of regular superficial ulcers,three were congestion and edema,a total of 4 of duodenal lesions,erosion,lack of regular superficial ulcers,a total of 9 were congestion,edema,dot,spot,and bar erosion,gastric sinus and gastric body will produce mucosal surface spot bleeding,a total of one is fusion patch bleeding lesions.Conclusion:In the early clinical diagnosis of abdominal anaphylactoid purpura,gastroscopy combined with color Doppler ultrasonography is a better method,combined examination of the two can facilitate early coconsultation of abdominal allergic purpura and provide guidance for physician treatment.
