超声产前检查在胎儿肢体畸形中的价值分析

摘　　要：目的:关于产前超声检查应用的胎儿肢体畸形诊断当中的价值研究。方法:选择2019年3月~2020年7月来本院进行产前超声检查的2000例孕妇作为调查对象,对所有研究对象跟踪随访到分娩,总结在胎儿肢体畸形检验方面应用产前超声的价值。结果:①产前超声检查显示胎儿的肢体畸形大多出现在孕周时间为(20.64±6.45)周这一阶段,最晚的确诊孕周为36周。2000例研究对象当中存在胎儿肢体畸形52例,经产前超声检查出48例,漏诊4例,产前超声的诊断率为92.31%。本研究存在1例手指畸形、1例脚趾畸形的胎儿漏诊,2例在产前超声检查中未发现肢体异常,但正常分娩以后发现肢体畸形,其中1例产前超声检查存在脊柱裂,引产后发现合并六指畸形,1例经产前超声诊断为体-蒂异常,引产后合并畸形脚;②产前超声诊断胎儿肢体畸形的阳性预测值为95.83%、阴性预测值为99.69%、敏感度为88.46%、特异度为99.9%、准确度为99.6%、约登指数为88.36%、误诊率为0.1%、漏诊率为11.54%。结论:应用产前超声对胎儿肢体畸形进行诊断,可发挥理想的诊断作用,可以将这种诊断手段当做是对胎儿骨骼发育的主要影像学评价手段,能够及早发现母体内胎儿肢体的异常状况,可以为临床尽早接受无效妊娠提供依据,有效降低新生儿缺陷的发生率。Objective:To study the value of prenatal ultrasound in the diagnosis of fetal limb malformations.Methods:In this study,2000 pregnant women who came to our hospital for prenatal ultrasound examination from March 2019 to July 2020 were selected as the survey subjects.All subjects were followed up to delivery,and the value of prenatal ultrasound in the examination of fetal limb malformations was summarized.Results:①Prenatal ultrasound examination showed that most of the fetal limb deformities appeared in the gestational period(20.64±6.45),and the latest diagnosis was 36 weeks.Among the 2000 study subjects,52 cases had fetal limb malformations,48 cases were detected by prenatal ultrasound,and 4 cases were missed.The diagnosis rate of prenatal ultrasound was 92.31%.In this paper,there is 1 case of finger malformation and 1 case of toe malformation.No limb abnormalities were found in prenatal ultrasound examination in 2 cases,but limb abnormalities were found after normal delivery.1 case of prenatal ultrasound examination had spina bifida,which was found after induction of labor.Combined with six-finger malformation,1 case was diagnosed as abnormal body-pedicle by prenatal ultrasound,combined with deformed foot after induction of labor;②Prenatal ultrasound diagnosis of fetal limb malformation had a positive predictive value of 95.83%,a negative predictive value of 99.69%,and sensitivity.It was 88.46%,the specificity was 99.9%,the accuracy was 99.6%,the Youden index was 88.36%,the misdiagnosis rate was 0.1%,and the missed diagnosis rate was 11.54%.Conclusion:The application of prenatal ultrasound in the diagnosis of fetal limb malformations can play an ideal diagnostic role.This diagnostic method can be used as the main imaging evaluation method for fetal skeletal development,which can detect abnormalities of the fetal limbs in the mother as early as possible.It can provide a basis for clinical acceptance of invalid pregnancy as soon as possible,and effectively reduce the incidence of neonatal defects.
