腹腔镜与开放式胰十二指肠切除术治疗壶腹周围癌疗效分析

摘　　要：目的:分析并比较腹腔镜与开放式胰十二指肠切除术治疗壶腹周围癌的临床治疗效果。方法:纳入2018年1月~2020年12月本院收治的壶腹周围癌患者68例为研究对象,其中34例应用腹腔镜手术设作观察组,34例应用开放胰十二指肠切除术设作对照组,比较两组疗效。结果:观察组与对照组,对比手术时间、术后进流食时间、住院时间有统计学差异(P<0.05);而术中输血量、术后排气时间无统计学差异(P>0.05);观察组术后并发症发生率41.2%低于对照组62.2%,有统计学差异(P<0.05)。结论:腹腔镜用于壶腹周围癌治疗,可以有效缩短手术时间,促进患者术后及早进食,利于胃肠功能的恢复,从而缩短手术时间,减少并发症发生,手术安全性较高。Objective:To analyze and compare the clinical effects of laparoscopic and open pancreaticoduodenectomy in the treatment of periampullary carcinoma.Methods:A total of 68 patients with periampullary carcinoma were included as the research objects.The data collection period was from January 2018 to December 2020.Among them,34 cases were treated with laparoscopic surgery as the laparoscopic group,and 34 cases were treated with open pancreaticoduodenum.The resection was set as an open group,and the efficacy of the two groups was compared.Results:The laparoscopic group and the open group had statistical differences in operation time,postoperative fluid feeding time,and hospital stay(P<0.05);but there was no statistical difference in intraoperative blood transfusion and postoperative exhaust time(P>0.05);the incidence of postoperative complications in the laparoscopic group was 41.2%lower than that of the control group 62.2%,which was statistically different(P<0.05).Conclusion:Laparoscopy for the treatment of periampullary carcinoma can effectively shorten the operation time,promote early postoperative eating,and be beneficial to the recovery of gastrointestinal function,thereby shortening the operation time and reducing the occurrence of complications.The operation is safer.
