摘 要:目的探讨2型糖尿病伴糖尿病足感染患者血清微小RNA(miRNA)表达与创面修复因子及免疫功能的关系。方法选择2020年1月-2020年12月吉林大学第一医院内分泌科收治的2型糖尿病伴糖尿病足感染患者80例为研究组,选择同期医院内分泌科进行治疗的2型糖尿病未伴任何感染患者60例为对照组。采用Trizol法提取血清RNA后采用实时荧光定量聚合酶链式反应(RT-PCR)检测miR-21、miR-132和miR-155水平;采用酶联免疫吸附法检测创面分泌物中血小板衍生生长因子BB(PDGF-BB)和碱性成纤维细胞生长因子(bFGF)水平,采用流式细胞术检测外周血CD_(4)^(+)、CD_(8)^(+)、自然杀伤细胞(NK)以及单核细胞水平。结果研究组PDGF-BB、bFGF水平高于对照组,CD_(4)^(+)/CD_(8)^(+)、NK和单核细胞水平低于对照组(P<0.05);Wanger分级3~4级患者miR-21、miR-132相对表达水平低于Wanger分级1~2级患者,miR-155相对表达水平高于Wanger分级1~2级患者(P<0.05);Wanger分级3~4级患者PDGF-BB、bFGF水平高于Wanger分级1~2级患者,CD_(4)^(+)/CD_(8)^(+)、NK和单核细胞水平低于Wanger分级1~2级患者(P<0.05)。结论miR-21、miR-132和miR-155与2型糖尿病合并糖尿病足感染患者病情分级及创面修复因子、细胞免疫水平有一定的相关性,可作为临床判断依据,其靶向治疗价值有待后续研究。OBJECTIVE To investigate the relationship between serum miRNA profile expression,wound repair factors and immune function in type 2 diabetes patients with diabetic foot infection.METHODS 80 type 2 diabetes patients with diabetic foot infection admitted to the Department of Endocrinology in First Hospital of Jilin University from Jan.2020 to Dec.2020 were selected as the study group,and 60 type 2 diabetes patients without any infection treated in the Department of Endocrinology of the hospital during the same period were selected as control group.After extracting serum RNA by Trizol method,real-time quantitive polymerase chain reaction(RT-PCR)was used to detect the levels of miR-21,miR-132 and miR-155;enzyme-linked immunosorbent assay was used to detect the levels of platelet-derived growth factor BB(PDGF-BB)and basic fibroblast growth factor(bFGF)in wound secretions,and flow cytometry was used to detect peripheral blood CD_(4)^(+),CD_(8)^(+),NK and monocyte levels.RESULTS The levels of PDGF-BB and bFGF in the study group were significantly higher than those in the control group,while the levels of CD_(4)^(+)/CD_(8)^(+),NK and monocytes were significantly lower than those in the control group(P<0.05).The relative expression of miR-21 and miR-132 in Wanger grade 3-4 patients were significantly lower than those in Wanger grade 1 to 2 patients,the relative expression of miR-155 was significantly higher than Wanger grade 1 to 2 patients(P<0.05).The PDGF-BB and bFGF levels of Wanger 3-4 patients were significantly higher than those of Wanger 1-2 patients,but the CD_(4)^(+)/CD_(8)^(+),NK and monocyte levels were significantly lower than those of Wanger grade 1-2 patients(P<0.05).CONCLUSION The miR-21,miR-132 and miR-155 had certain correlation with the disease grading,wound repair factors and cellular immune levels of type 2 diabetes patients with diabetic foot infection,which could be used as a basis for clinical judgment.The value of their targeted therapy needs follow up research.
