摘  要:目的研究CD40L蛋白、单核细胞趋化蛋白(MCP-1)联合肺部感染评分(CPIS)对新生儿感染性肺炎预后的评估价值。方法选取2017年7月-2018年7月在济南市第二妇幼保健院治疗的新生儿感染性肺炎患儿78例为肺炎组,另选取同期健康新生儿78名为正常组,采用酶联免疫吸附试验检测CD40L、MCP-1水平;CPIS对患儿进行肺部评分。采用受试者工作特征曲线(ROC)分析CD40L、MCP-1联合CPIS评分对新生儿感染性肺炎患儿预后的评估价值。结果与正常组相比,肺炎组CD40L、MCP-1水平和CPIS评分均升高,差异具有统计学意义(P<0.05)。有脓性痰、有融合片状浸润影、有呼吸暂停的患儿CD40L、MCP-1表达升高,差异具有统计学意义(P<0.05)。与预后结局良好组相比,不良预后结局组CD40L、MCP-1、CPIS评分表达量均升高,差异具有统计学意义(P<0.05)。与CD40L、MCP-1、CPIS评分相比,三项联合对新生儿感染性肺炎不良预后结局预测价值较高,具有统计学差异(P<0.05)。结论相比正常新生儿,CD40L、MCP-1在新生儿感染性肺炎患儿中异常高表达,CPIS评分较高,可用于临床上对新生儿感染性肺炎患儿预后的评估。OBJECTIVE To observe the values of CD40 L protein(CD40 L) and monocyte chemotactic protein(MCP-1) combined with pulmonary infection score(CPIS) in assessment of prognosis of neonates with infectious pneumonia. METHODS A total of 78 neonates with infectious pneumonia who were treated in Jinan Second Maternal and Child Health Hospital from Jul 2017 to Jul 2018 were assigned as the pneumonia group, meanwhile, 78 healthy neonates were chosen as the normal group. The levels of CD40 L and MCP-1 were detected by using enzyme-linked immunosorbent assay, the pulmonary scoring was carried out by CPIS. The values of CD40 L and MCP-1 combined with CPIS in assessment of prognosis of the neonates with infectious pneumonia were analyzed by means of receiver operating characteristic(ROC) curve. RESULTS The levels of CD40 L, MCP-1 and CPIS score were significantly higher in the pneumonia group than in the normal group(P<0.05). The expression levels of CD40 L and MCP-1 were significantly higher in the neonates with purulent sputum, fusion flake infiltration shadow and apnea(P<0.05). The expression levels of CD40 L, MCP-1 and CPIS score were significantly higher in the neonates with poor prognostic outcomes than in the neonates with favorable prognostic outcomes(P<0.05). The joint detection of CD40 L, MCP-1 and CPIS had significantly higher value in prediction of poor prognostic outcomes of the neonates with infectious pneumonia than the single detection did(P<0.05). CONCLUSION As compared with the healthy neonates, the neonates with infectious pneumonia show abnormal expression of CD40 L and MCP-1, with the CPIS score high, and it can be used for clinical assessment of prognosis of the neonates with infectious pneumonia.
