摘 要:目的探讨核因子-κB1(NF-κB1)启动子区-94ins/del ATTG基因多态性与寻常型天疱疮(PN)易感性及继发感染的关联。方法选择2019年9月-2020年10月中国人民解放军联勤保障部队第九〇三医院皮肤科收治的PN患者90例为研究组,选择同期医院进行体检健康人70名为对照组。针对NF-κB1启动子区-94ins/del ATTG设计特异性引物,采用聚合酶链式扩增反应(PCR)扩增目的片段,凝胶成像仪观察分型结果。结果研究组DD、DI基因型、D等位基因频率高于对照组,差异具有统计学意义(P<0.05);校正常量因素后,携带DD基因型是Ⅱ基因型发生PN的1.28倍,携带D等位基因是携带Ⅰ等位基因发生PN的1.31倍,均具有统计学意义(P<0.05);感染组DD基因型频率高于非感染组,差异具有统计学意义(P<0.05);校正常量因素后,携带DD基因型是Ⅱ基因型发生皮肤感染的1.35倍,具有统计学意义(P<0.05);DD型PN患者天疱疮面积指数(PDAI)评分高于DⅠ/Ⅱ型患者,两组性别、年龄、皮损部位数量、黏膜受累情况比较,无统计学差异。结论 NF-κB1启动子区-94ins/del ATTG基因多态性与本地区PN易感性、继发皮肤感染及PDAI评分有关,其机制有待后续研究。OBJECTIVE To explore the relationship between the nuclear factor kappa B(NF-κB1) promoter region-94 ins/del ATTG gene polymorphism and the susceptibility to pemphigus vulgaris and secondary infection. METHODS A total of 90 patients with pemphigus vulgaris who were treated in dermatology department of the 903 rd Hospital of the chinese People’s Liberation Army of the Joint Logistic Support Force from Sep 2019 to Oct 2020 were assigned as the study group, meanwhile, 70 healthy people who received physical examination were set as the control group. Specific primers were designed for-94 ins/del ATTG in the promoter region of NF-κB1, and the target fragment was amplified by polymerase chain reaction, and the typing results were observed by a gel imager. RESULTS The frequencies of genotypes DD, DI and allele D of the study group were significantly higher than those of the control group(P<0.05). After the constant factors were adjusted, the incidence of PN was as 1.28 times in the patients carrying with genotype DD as in the patients carrying with genotype Ⅱ, the incidence of PN was as 1.31 times in the patients carrying with allele D as in the patients carrying with allele Ⅰ, and there were significant differences(P<0.05). The frequency of genotype DD of the infection group was significantly higher than that of the non-infection group(P<0.05). After the constant factors were adjusted, the incidence of skin infection was as 1.35 times in the patients carrying with genotype DD as in the patients carrying with Ⅱ, and there were significant differences(P<0.05). The pemphigus area index(PDAI) score of the PN patients with DD genotype was higher than that of the patients with genotype DⅠ/Ⅱ;there were no significant differences in the gender, age, number of damaged skin sites and mucosal involvement between the two groups. CONCLUSION The gene polymorphisms of-94 ins/del ATTG in the promoter region of NFκB1 is associated with the susceptibility to PN, secondary skin infection and PDAI score, and the mechanisms n
