摘 要:膝关节具有多样且复杂的运动形式,是人体承重最大的关节,最易导致关节软骨损伤。由于其再生修复的能力很弱,故其损伤后很难自身修复,进而导致骨关节炎的发生。膝关节软骨损伤后目前主要有以下的手术治疗方式:(1)自体骨软骨移植技术(马赛克移植技术);(2)同种异体骨软骨移植技术;(3)自体软骨细胞移植;(4)合成或生物支架植入(组织工程技术);(5)微骨折技术;(6)粉碎软骨修复技术;(7)生物制剂辅助治疗。通过选择性运用上述治疗可以在短期内改善临床症状,延迟或者避免行关节置换手术,本文将对其一一进行综述。Knee joint has various and complex movement forms,which is the joint with the largest bearing capacity of human body and is most prone to articular cartilage damage.Due to its weak regeneration and repair ability,it is difficult to repair itself after injury,leading to the occurrence of osteoarthritis.At present,there are mainly the following surgical treatment methods after cartilage injury of knee joint:①Autogenous osteochondral transplantation technology(Mosaic transplantation technology);②Osteochondral allograft technology;③Autologous chondrocyte implantation;④Synthetic or biological scaffold implantation(tissue engineering technology);⑤Micro-fracture technology;⑥Crushed cartilage repair technology;⑦Adjuvant therapy with biological preparation.Selective use of these therapies can improve clinical symptoms in the short term,delaying or avoiding joint replacement surgery.This article will review them one by one.
