治疗风湿病上市中成药的处方规律研究

摘　　要：【目的】探索治疗风湿病上市口服中成药的处方规律,挖掘数据、传承经验,为临床用药及新药开发提供参考。【方法】通过检索国家食品药品监督管理局网站及《新编国家中成药》中收录的已上市的中成药,建立数据库,进行分类、排序,总结其处方规律,包括处方功效、药物频率、药物分类、性味归经及毒性药物的使用情况。【结果】通过检索,共纳入166种中成药处方,中药267种,药物使用频次共1881次。针对病因,从风湿寒论治最多,祛风湿药、活血药和补益药为使用频次最高的种类,其中使用频次和频率最高的药物为当归、牛膝和威灵仙;所涉及药物性味温辛苦温、归肝脾肾经的药物使用频次最高。毒性药物使用种类为27种,其中川乌、马钱子和草乌为高频使用药物;利用关联规则分析得出常用组合16个,核心药物13味;利用复杂熵层次聚类算法进一步挖掘,演化出16个核心组合,8个新处方。【结论】本研究客观地揭示了风湿类疾病病位在肝脾肾,以扶正、祛邪、活血为主要手段,为临床辨证用药及药物研究提供了证据。【Objective】To explore the prescription rules of marketed oral Chinese patent medicines for rheumatism,mining data and passing on experience,providing reference for clinical drug use and new drug development.【Methods】By searching the website of STATE Food and Drug Administration and the listed Proprietary Chinese medicine in The New Edition of National Proprietary Chinese Medicine,a database was established,classified and sorted,and the prescription rules were summarized,including prescription efficacy,drug frequency,drug classification,sexual taste and toxic drug use.【Results】Through retrieval,a total of 166 proprietary Chinese medicine prescriptions and 267 traditional Chinese medicines were included,with a total of 1881 drug use frequency.In terms of etiology,the treatment of wind-dampness and cold was the most common,and wind-dampness dispelling drugs,blood-activating drugs and tonifying drugs were the most frequently used,among which the drugs with the highest frequency and frequency were Angelica,Achyrantha bidentata and Weilingxian.The frequency of drug use was the highest in the cases of taste temperature,spleen and kidney meridian.There were 27 kinds of toxic drugs,among which Monkshood,Strychnine and Monkshood were frequently used.By association rule analysis,16 commonly used combinations and 13 core drugs were obtained.The complex entropy hierarchical clustering algorithm was used to further mine,and 16 core combinations and 8 new prescriptions were evolved.【Conclusion】This study was objectively revealed that rheumatic diseases are located in liver,spleen and kidney,and the main means of strengthening,removing pathogenic factors and promoting blood circulation are used to provide more certain evidence for clinical syndrome differentiation and drug research.
