低分子肝素钙联合压力泵预防老年髋骨骨折并发DVT的疗效评价

摘　　要：【目的】评价低分子肝素钙辅以气压泵预防老年髋骨骨折并发下肢深静脉血栓(DVT)的疗效。【方法】选择2018年4月至2021年5月于我院住院的周围性骨折患者82例,按照随机数字表法分为两组,对照组予以单纯的低分子肝素钙进行治疗,观察组予以低分子肝素钙联合压力泵进行治疗,对比两组DVT发生率、肿胀后下肢周径差值、血常规和凝血功能变化。【结果】干预后7 d、10 d,两组的DVT发生率均低于干预前(P<0.05);观察组干预后10 d的DVT发生率显著低于对照组(P<0.05)。干预后10 d的下肢周径差值与干预前相比显著减小(P<0.05);而观察组干预后7 d、10 d均显著低于干预前(P<0.05);观察组干预后10 d下肢周径差值显著低于对照组(P<0.05)。观察组干预后10 d血小板显著低于对照组(P<0.05)。干预后7 d、10 d,对照组的凝血酶原时间(PT)显著小于观察组(P<0.05);干预后10 d,观察组活化部分凝血活酶时间(APTT)显著低于对照组(P<0.05)。【结论】低分子肝素钙辅以气压泵能显著降低髋关节周围骨折术后DVT发生率,有利于改善患者下肢肿胀,具有较好的积极作用。【Objective】To evaluate the efficacy of low molecular weight heparin calcium combined with pneumatic pump in the prevention of hip fracture complicated with deep venous thrombosis(DVT)in the elderly.【Methods】A total of 82 patients with peripheral fractures hospitalized in our hospital from April 2018 to May 2021 were randomly divided into two groups.The control group was treated with low molecular weight heparin calcium alone,and the observation group was treated with low molecular weight heparin calcium combined with pressure pump.1~10 days after treatment,the incidence of DVT,peripheral diameter difference of lower limbs after swelling,blood routine and coagulation function of the two groups were compared.【Results】After 7 and 10 days of intervention,the incidence of DVT in the two groups was lower than that before intervention(P<0.05);The incidence of DVT in the observation group after 10 days of intervention was significantly lower than that in the control group(P<0.05).The peripheral diameter difference of lower limbs 10 days after intervention was significantly lower than that before intervention(P<0.05);After 7 and 10 days of intervention in the observation group,it was significantly lower than that before intervention(P<0.05);The peripheral diameter difference of lower limbs in the observation group was significantly lower than that in the control group 10 days after intervention(P<0.05).The platelet value of the observation group was significantly lower than that of the control group(P<0.05).At 7 and 10 days after intervention,the prothrombin time(PT)of the control group was significantly lower than that of the observation group(P<0.05);At 10 days after intervention,activated partial thromboplastin time(APTT)in the observation group was significantly lower than that in the control group(P<0.05).【Conclusion】Low molecular weight heparin calcium combined with air pressure pump can significantly reduce the incidence of DVT after operation of hip fracture,which is conducive to improve the swelli
