基于英国患者可及性方案的医保谈判协议类型比较研究

摘　　要：目的:介绍英国患者可及性方案,分析其协议类型、特点及在我国实施的可行性,为我国医保药品价格谈判协议的设计提供借鉴。方法:通过文献研究和政策分析法,对英国患者可及性方案的签订主体、约定方式和协议类型进行梳理与分析。结果:英国通过患者可及性方案建立了多样化的药品价格谈判协议,并借助价格保密条款分担创新药在疗效及费用方面的不确定性,缓解财政支出压力,维系全球价格体系的稳定。结论:建议我国在医保药品谈判中探索多样化的价格协议,同时完善协议价格保密机制,促进谈判双方的合作共赢。Objective:To introduce the Patient Access Schemes in the UK,analyzing its agreement types,characteristics and feasibility of implementation in China,and to provide a reference for the design of China’s medical insurance drug price negotiation agreements.Methods:Through literature research and policy analysis,the contracting subject,agreement method and schemes types of the Patient Access Schemes were sorted and analyzed.Results:The UK has established diversified drug price negotiation agreements through the Patient Access Schemes.With the help of price confidentiality clauses,Patient Access Schemes share the uncertainty of the efficacy and cost of innovative drugs,ease the pressure on financial expenditures,and maintain the stability of the global price system.Conclusion:It is recommended that China should explore diversified price agreements in the negotiation of medical insurance drugs,improve the price confidentiality mechanism of the agreements,and promote the win-win cooperation between the two parties in the negotiation.
