卡铂致过敏性休克22例文献分析

摘　　要：目的分析注射用卡铂致过敏性休克的一般规律及特点,为临床安全用药提供参考。方法通过检索中国知网全文数据库(CNKI)、万方数据库及维普中文期刊全文数据库等数据库,收集卡铂致过敏性休克病例,按照患者性别与年龄、原患疾病、药物不良反应史、过敏性休克发生时间、用药疗程、累及器官与临床表现及转归等进行统计分析。结果原患疾病主要为卵巢癌(9例,40.91%)和肺癌(9例,40.91%),有既往过敏史的7例(31.82%);22例中20例为静脉注射液,2例为灌注给药,过敏性休克主要发生在10 min内(14例,63.64%),多发生于第8次化疗(11例,50.00%)后;主要表现为循环系统损害(79例次,49.38%),无死亡病例。结论卡铂致过敏性休克多发生于多次化疗后及前期出现轻微不良反应的患者,临床用药应对该类患者加强用药监护,保障患者用药安全。Objective To analyze the general rules and characteristics of anaphylactic shock caused by carboplatin for injection,and to provide references for clinical safe medication.Methods The cases of carboplatin-induced anaphylactic shock were collected by searching databases such as China National Knowledge Infrastructure(CNKI),Wanfang Database and VIP Chinese Journal Full-text Database(VIP).The gender and age of the patient,the underlying disease,the history of adverse drug reactions,the time of occurrence of anaphylactic shock,the course of medication,organs involved and clinical manifestations and outcomes were statistically analyzed.Results The primary diseases were mainly ovarian cancer(9 cases,40.91%)and lung cancer(9 cases,40.91%),7 cases(31.82%)with a history of allergies.Among 22 cases,20 cases were intravenous injection,2 cases were infusion administration,anaphylactic shock mainly occurred within 10 minutes(14 cases,63.64%),and mostly occurred after the 8th chemotherapy(11 cases,50.00%).The main manifestation was circulatory system damage(79 cases,49.38%),and no death cases.Conclusion Carboplatin-induced anaphylactic shock mostly occurs in patients with mild adverse reactions after multiple chemotherapy and early stage.Clinical medication should strengthen medication monitoring for such patients to ensure patient medication safety.
