子宫肌瘤患者腹腔镜手术后恶心呕吐发生的相关因素

摘　　要：目的探究子宫肌瘤患者腹腔镜手术后发生恶心呕吐(PONV)的影响因素。方法选取2019年1月至2021年8月期间于商丘市中医院行腹腔镜手术治疗的105例子宫肌瘤患者为研究对象,记录患者术后PONV发生情况及基线资料,将可能的影响因素纳入,分析可能导致子宫肌瘤患者腹腔镜手术后发生PONV的相关因素。结果105例子宫肌瘤腹腔镜手术后PONV 40例,发生率为38.10%(40/105);发生组PONV史、手术时间、术前静脉补液量及术后使用阿片类止痛药物与未发生组比较,差异有统计学意义(P<0.05);经logistic回归分析结果显示,术前静脉补液量少、PONV史、手术时间长及术后使用阿片类止痛药物是子宫肌瘤患者腹腔镜手术后发生PONV的危险因素(OR>1,P<0.05)。结论子宫肌瘤患者腹腔镜手术后发生PONV与PONV史、手术时间长、术前静脉补液量少及术后使用阿片类止痛药物等因素有关。Objective To explore the influencing factors of postoperative nausea and vomiting(PONV)in patients with hysteromyoma after laparoscopic surgery.Methods A total of 105 patients with hysteromyoma who underwent laparoscopic surgery in Shangqiu Hospital of Traditional Chinese Medicine from January 2019 to August 2021 were selected as the research subjects,the occurrence of PONV and baseline data of patients were recorded,included possible influencing factors,the related factors that might lead to PONV after laparoscopic surgery in patients with hysteromyoma were analyzed.Results Among 105 cases of hysteromyoma after laparoscopic surgery,PONV occurred in 40 cases,with an incidence of 38.10%(40/105).There were statistical differences on PONV history,operation time,preoperative intravenous rehydration and postoperative use of opioid analgesics between the occurrence group and the non occurrence group(P<0.05).Through logistic regression analysis,the results showed that the risk factors of PONV in patients with hysteromyoma after laparoscopic surgery were less preoperative venous rehydration,history of PONV,long operation time and postoperative use of opioid analgesics(OR>1,P<0.05).Conclusion PONV in patients with hysteromyoma after laparoscopic surgery is related to PONV history,long operation time,less preoperative venous rehydration and postoperative use of opioid analgesics.
