摘 要:目的探讨磁共振扩散张量成像(DTI)定量分析联合扩散加权成像(DWI)在脑胶质瘤术前分级诊断中的应用价值。方法收集2018年10月至2020年10月南阳市第二人民医院收治的74例脑胶质瘤患者临床资料,患者均在术前接受DTI定量分析联合DWI检查,以手术病理检查结果为标准,比较不同脑胶质瘤分级患者DTI定量分析联合DWI检测参数[表现扩散系数(ADC)、各向异性分数(FA)、局部脑血流量(rCBF)],并绘制受试者ROC曲线,分析DTI定量分析联合DWI在脑胶质瘤术前分级诊断中应用情况。结果74例脑胶质瘤患者中,高级别胶质瘤44例(59.46%),低级别胶质瘤30例(40.54%);高级别组ADC、FA低于低级别组,rCBF高于低级别组,差异有统计学意义(P<0.05);绘制ROC曲线图,结果显示,ADC、FA、rCBF单独及联合评估脑胶质瘤术前分级的AUC均>0.7,当取最佳截断值时可获得最佳预测价值。结论DTI定量分析联合DWI评估脑胶质瘤患者术前分级中的价值较高。Objective To investigate the application value of quantitative analysis of magnetic resonance diffusion tensor imaging(DTI)combined with diffusion weighted imaging(DWI)in the preoperative grading diagnosis of glioma.Methods The clinical data of 74 patients with glioma treated in Nanyang Second People’s Hospital from October 2018 to October 2020 were collected,all patients were examined by DTI quantitative analysis combined with DWI before operation.The results of surgical and pathological examination were taken as the standard,DTI quantitative analysis combined with DWI detection parameters[apparent diffusion coefficient(ADC),anisotropy fraction(FA),regional cerebral blood flow(rCBF)]were compared in patients with different glioma grades,and the subject ROC curve was drawn,the application of DTI quantitative analysis combined with DWI in preoperative grading diagnosis of glioma was analyzed.Results Among 74 patients with glioma,44 cases(59.46%)had high-grade glioma and 30 cases(40.54%)had low-grade glioma.ADC and FA in high-grade group were lower than those in low-grade group,and rCBF was higher than that in low-grade group(P<0.05).The ROC curve was drawn,and the results showed that the AUC of ADC,FA and rCBF alone and in combination for evaluating preoperative grading of glioma were>0.7,when the best cut-off value was taken,the best prediction value could be obtained.Conclusion DTI quantitative analysis combined with DWI is of high value in evaluating preoperative grading of patients with glioma.
