乙型肝炎患者肝超声弹性值、肝脂肪含量与肝纤维化血清学指标的相关性

摘　　要：目的探讨乙型肝炎患者肝超声弹性值、肝脂肪含量与肝纤维化血清学指标的相关性。方法88例乙型肝炎患者行超声弹性成像及MRI,分析肝超声弹性值、肝脂肪含量与肝纤维化血清学指标[纤维连接蛋白(FN)、透明质酸(HA)、3型前胶原N端肽、Ⅳ型胶原(C-Ⅳ)、层粘蛋白、人结合珠蛋白]相关性。结果肝超声弹性值、肝脂肪含量分别为(17.5±13.2)kPa、(19.6±12.2)%。肝超声弹性值与C-IV、HA呈正相关(Y=2.9+0.08 C-IV+0.04HA,P<0.01),而肝脂肪含量与FN和C-IV呈正相关(Y=-0.06+0.0007 FN+0.0005 C-Ⅳ,P<0.01)。结论乙型肝炎患者肝超声弹性值与血清C-IV和HA呈正相关,而肝脂肪含量与血清FN和C-Ⅳ呈正相关.Objective To investigate the correlation between hepatic ultrasound elastic value(UEV),fat content and serological indexes of hepatofibrosis in hepatitis B.Methods Eighty-eight patients with hepatitis B underwent ultrasonic elastography and magnetic resonance imaging(MRI).The correlation between hepatic UEV,fat content and serological indexes[fibronectin(FN),hyaluronic acid(FA),procollagen type 3 N-terminal peptide,collagenⅣ(C-Ⅳ),laminin and human haptoglobin]of hepatofibrosis was analyzed.Results Hepatic UEV and fat content were(17.5±13.2)k Pa and(19.6±12.2)%,respectively.Hepatic UEV was positively correlated with C-Ⅳand HA(Y=2.9+0.08 C-IV+0.04 HA,P<0.01),while fat content with FN and C-Ⅳ(Y=-0.06+0.0007 FN+0.0005 C-Ⅳ,P<0.01).Conclusion There are positive correlations between hepatic UEV and C-Ⅳand HA and between hepatic fat content and FN and C-Ⅳin patients with hepatic B.
