摘 要:目的探讨烧伤后患者增生性瘢痕组织中铁代谢相关蛋白的表达情况。方法选择2016—2018年在石家庄市第一医院烧伤整形外科住院的15例烧伤患者作为增生性瘢痕组,选取同期15例因外伤或行美容手术者作为正常组。术前均采集血样,检测2组血清铁、总铁结合力、不饱和铁结合力、转铁蛋白饱和度及转铁蛋白含量。正常组采集正常皮肤组织标本,增生性瘢痕组采集瘢痕处及其自身正常皮肤组织标本,应用电感耦合质谱分析仪测定皮肤组织标本中铁含量,采用蛋白免疫印记法(Western blot)检测增生性瘢痕组织和自身正常皮肤组织中转铁蛋白受体1(TfR1)、铁蛋白(FTL)、二价金属离子转运蛋白[DMT1,包括DMT1(+IRE)和DMT1(-IRE)]、膜铁转运蛋白(FPN1)表达情况。结果正常组和增生性瘢痕组血清铁、总铁结合力、不饱和铁结合力、转铁蛋白饱和度及转铁蛋白含量比较差异均无统计学意义(P均>0.05)。正常组皮肤组织中和增生性瘢痕组自身正常皮肤组织中铁含量比较差异无统计学意义(P>0.05);增生性瘢痕组增生性瘢痕组织中铁含量及FTL、DMT1(+IRE)、DMT1(-IRE)、FPN1表达量均明显高于自身正常皮肤组织(P均<0.05),TfR1表达量明显低于自身正常皮肤组织(P<0.05)。结论烧伤后增生性瘢痕患者全身处于铁稳态,瘢痕局部铁元素沉积过多,细胞主要依赖于TfR1以及FPN1的作用而非DMT1(+IRE)和DMT1(-IRE)调节铁的摄取和释放。Objective It is to investigate the expression of iron metabolism-related proteins in hypertrophic scar tissue of patients after burn.Methods Fifteen burn patients who were hospitalized in the Department of Burns and Plastic Surgery of Shijiazhuang People's Hospital from 2016 to 2018 were selected as the hypertrophic scar group,and 15 patients with trauma or cosmetic surgery during the same period were selected as the normal group.The blood samples of all the subjects were collected before surgery to detect the contents of serum iron,total iron binding capacity,unsaturated iron binding capacity,transferrin saturation and transferrin in the two groups.Normal skin tissue samples were collected in the normal group,and the scar and its own normal skin tissue samples were collected in the hypertrophic scar group.Inductively coupled mass spectrometry was used to determine the iron content in the skin tissue samples,and Western-blot was used to detect the expression of transferrin receptor 1(TfR1),ferritin(FTL),and divalent metal ion transporter[DMT1,including DMT1(+IRE)and DMT1(-IRE)],ferroportin(FPN1)in hypertrophic scar tissue and normal skin tissue.Results There was no statistically significant differences in the contents of serum iron,total iron binding capacity,unsaturated iron binding capacity,transferrin saturation and transferrin between the normal group and the hypertrophic scar group(all P>0.05).There was no significant difference in the iron content between the skin tissue of the normal group and the normal skin tissue of the hypertrophic scar group(P>0.05).The iron content and the expression of FTL,DMT1(+IRE),DMT1(-IRE)and FPN1 in the hypertrophic scar tissue of the hypertrophic scar group were significantly higher than those in the normal skin tissue(P<0.05),and the expression of TfR1 was significantly lower than the normal skin tissue.Skin tissue(P<0.05).Conclusion The patients with hypertrophic scars after burns are in iron homeostasis,and there is excessive iron deposition in the scar.The cells mainly re
